You will...

• Provide one-on-one skilled nursing care and treatment to patients in their homes.
• Assist registered nurses in assessing patients' conditions and contributing to care planning.
• Demonstrate expertise in home health and discipline-specific patient care skills.
• Perform comprehensive patient assessments and collaborate with multidisciplinary care teams.
• Monitor vital signs, administer treatments, and educate patients and families on medical conditions.
• Manage patient episodes according to protocol.
• Complete documentation accurately and timely.

You have...

• Current and unencumbered LPN license
• 1+ year(s) of Certified Home Health experience as an LPN
• Current CPR certification
• Comfortable with technical tools and EMR for visit documentation
• Reliable transportation, driver's license, and car insurance
• Proof of Immunization/Exemption for Flu and COVID-19
• TB Test or Chest X-Ray
