Job Summary
Responsible for assigning diagnostic and procedural codes to patient charts of moderate to high complexity using ICD-10-CM, CPT and HCPCS or any other designated coding classification system in accordance with coding rules and regulations. Abstracting required clinical information from the medical record.

Essential Duties And Responsibilities

• Thoroughly reviews the medical record as part of the coding process in order to assign and report the most appropriate codes.
• Assigns and report codes, without physician or clinician consultation, to diagnoses only if these diagnoses are specifically documented by the physician in the body of the medical record and the documentation is clear and concise
• Assesses clinical documentation to assure that it supports the diagnosis codes selected.
• Consults the clinician via electronic alerts for clarification when conflicting or ambiguous documentation is present, and for the clinician's acceptance or declination of changes.
• Revises codes reported when the physician or clinician disagrees with the code selection.
• Provides coding and classification systems education to any and all clinicians when opportunities arise (e.g. provide pertinent official coding guidelines when the clinician questions a code assignment).
• Seeks clarification, using all available references, when a specific code for a listed condition cannot be located.
• Responsible for daily completion of assigned coding tasks.
• Responsible to follow workflow process exactly as outlined in training material without deviation.
• Productivity as defined upon hire must be met by second month post orientation.
• Perform other Duties as assigned timely and effectively.
• Participates in staff meetings and educational sessions.
• Coding Quality: Demonstrates ability to achieve accuracy and consistency in the selection of principal and secondary diagnoses and procedures. Demonstrates ability to achieve accuracy and consistency in abstracting elements defined by per facility.
• Goal: Average coding quality standard of =>95% accuracy per monitoring period.

• ​​​Coding Labor Productivity: Meets and/or exceeds Vee Healthtek's coding productivity guidelines.
• Professional Development: Stays current with AHA Official Coding and Reporting Guidelines, CMS and other agency directives for ICD-10-CM and CPT coding. Attends mandatory coding seminars on an annual basis (IPPS and OPPS, ICD-10-CM and CPT updates) for inpatient and outpatient coding.
• Communicates and resolves coding issues (lacking documentation, physician queries, etc.) for appropriate follow-up and resolution.
• Adheres to all official coding guidelines as approved by the Public Health Service and the Centers for Medicare and Medicaid Services.
• Selects the principal diagnosis according to CMS definitions, and observe sequencing rules identified by the ICD guidelines.

• Proficient in outpatient diagnosis coding guidelines.
• Proficient in CPT/HCPCS code assignment including Evaluation & Management facility coding guidelines.
• Ability to establish and maintain effective working relationships as required by the duties of the position.
• Adept at comparing documentation, code assignment and charge in the financial system for accuracy and completeness and elevating concerns to the appropriate manager.
• Ability to establish and maintain effective working relationships as required by the duties of the position.
• Ability to concentrate and accomplish tasks with explicit accuracy.
• Ability to use office equipment and automated systems/applications/software at an acceptable level of proficiency.
• Functional knowledge of facility EMR, encoder and other support software

Education / Experience

• One year of experience performing medical record coding in acute care setting preferred.
• High school graduate or equivalent is required.
• Completion of basic coding course (academic, seminar, workshop or facility-based), including medical terminology and basic anatomy and physiology, or an equivalent combination of education and experience also required.

Certificates, Licenses, Registrations

• Required: AHIMA or AAPC approved credential

Salary: $25.00 - $28.00/hour depending on experience. This position is eligible for full health insurance including medical/dental/vision, PTO, and a 401k match!

*A Coding Assessment Test will be administered before initial pre-screen.

Travel Requirements: Fully remote/home-based office
