A rewarding opportunity to bring quality care to an underserved population in Providence:

• Practice is weekday outpatient primary care – 4 days in clinic; one day of admin remote.
• Collaborate with a terrific team of physicians from all specialties, integrated behavioral health clinicians, community health advocates, nurse practitioners, nurse case managers, and medical leadership.
• Lead your team – a dedicated bilingual Medical Assistant, and RN
• Enhance your cultural competency working with our diverse multi-cultural patient population.

• We have 550 employees serving 80,000 patients at our neighborhood-based clinics in Providence.
• Our clinics are Patient-Centered Medical Homes. Resources include community advocates, on-site Integrated Behavioral Health counselors, Clinical Pharmacists.
• We have 8 primary care clinics; a clinic dedicated to homeless healthcare; and a clinic dedicated to patients with severe-persistent mental illness. In addition, PCHC specialty clinics include Podiatry, Optometry, Dental, Rheumatology; Asthma-Allergy.

Qualifications and Skills:

Physicians:

• Board-certified or board-eligible in Internal Medicine or Family Medicine
• Licensed/eligible in Rhode Island
• Basic Life Support (BLS) certification
• Multi-lingual highly valued.

Nurse Practitioners: 3+ years of Primary Care Experience

• Interest in community health and patient-centered care
• Licensed/eligible in Rhode Island
• Basic Life Support (BLS) certification
• Multi-lingual highly valued.
