About This Position
Program Coordinator
The Program Coordinator (PC) holds the responsibility of overseeing, providing leadership, and ensuring the care of individuals served within their assigned site. Their role involves safeguarding the daily well-being and safety of the clients under their care, conducting employee training, ensuring adherence to client care plans, and actively assisting clients with all activities of daily living (ADLs), personal care needs, program implementation, and data collection. Moreover, the Program Coordinator is accountable for maintaining adequate client supervision, as well as supporting household maintenance and housekeeping tasks.

Desirable Qualifications:
- Experience working with individuals with a vulnerable population in a healthcare oversight capacity.
- Leadership experience and/or training in disability services.
- Strong ability to support and collaborate with a team in understanding client healthcare needs, management of care plans, and working closely with the IDT and BrightPath Management team.
- A minimum of 50 hours of education and training related to human services and disabilities.
- Must pass a Department of Human Services background check and maintain qualified status.
- Must have a current valid driver's license and insurance as required by state law.
- Proficiency in computer usage aligned with client healthcare needs and overall agency standards.
Direct Client Care and Program Responsibilities:
- Provide direct care and assistance to clients based on their individual Support Plan and Support Plan Addendum.
- Support clients with daily cares, medical needs, activities of daily living (ADLs), and mental health supports.
- Implement person-centered programs in alignment with the Service and Support Plan, Support Plan Addendum, and Individual Abuse Prevention Plan.
- Assist in assessing and developing client program goals, monitoring progress, and collecting relevant data.
- Complete and distribute quarterly progress reports for clients.
- Participate in interdisciplinary team (IDT) client meetings to contribute to client care planning and coordination.
- Take responsibility for meal planning, household supplies, and grocery shopping, ensuring clients receive three nutritious and balanced meals in accordance with CRS licensing and 245D standards.
- Oversee the cleanliness of the site and ensure that cleaning tasks are completed during each shift by assigned staff members.
- Provide oversight and management of overall healthcare for clients, including medication orders, inventory, and coordination of Medication Administration Review with the Registered Nurse (RN).
- Track medical appointments, update medication records in Therap E-MAR via the Pharmacy Message interface and ensure accurate documentation of after-visit summaries.
- Conduct regular reviews of Therap Medication Administration Records (MARs) to ensure accuracy and completeness.
- Rectify any instances of blank spots or missing information in the MARs, maintaining up-to-date and comprehensive records.
- Collaborate with relevant team members to address discrepancies or issues identified in medication administration and documentation.
- Adhere to organization's documentation timelines and reporting requirements for medication management.
- Work closely with the team to ensure a proper understanding and implementation of client care plans and active treatment plans.
- Train new staff and assist team members in understanding client care plans, ADLs, and active treatment plans.
- Collaborate with the Lead Designated Coordinator to maintain and organize house and client binders according to agency expectations and licensing standards.
- Collaborate with the Scheduling Coordinator to ensure adequate staffing coverage.
- Ensure compliance with licensing standards, including quarterly fire and weather drills.
- Complete quality improvement tools and client attendance records.
- Document ISP program data and daily log information accurately and in a timely manner.
- Regularly review ISP data to verify its accuracy and compliance with established standards.
- Ensure that each shift has the correct number of ISP data entries based on specific program requirements and staffing ratio.
- Address any concerns or discrepancies raised in staff event descriptions promptly and appropriately.
- Follow up with staff regarding behavioral issues, incidents, and safety concerns, ensuring appropriate documentation and reporting procedures are followed.
- Complete all mandatory training as determined by BrightPath and 245D regulations.
- Read and follow company policies and procedures.
- Collaborate with the team to fulfill other assigned duties or address emerging needs.
- In collaboration with the Designated Coordinator assist with facilitating regular staff meetings to foster effective communication and collaboration.
- Stand/walk for up to 8 hours per shift
- Support individuals with mobility needs, including transfers
- Lift and move up to 50 lbs infrequently
- Perform physical assistance tasks, including bending, kneeling, and reaching
- Climb stairs occasionally
- Ability to implement approved physical interventions for behaviors when necessary
- Physical stamina to maintain safe environments during behavioral incidents
- Ensure proper allocation of all Divvy transactions and maintain accurate records.
- Follow guidelines and policies for uploading receipts and verifying purchases made.
- Adhere to budgeting guidelines and use the company credit card strictly for authorized purposes.
- Allocate client food-related expenses appropriately and maintain receipts to support the purchasing of necessary items.
