This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for an Oncology Nurse Navigator based in the United States.
As an Oncology Nurse Navigator, you will provide compassionate, clinically focused support to members throughout their cancer journey.
Working remotely by phone and email, you will help members navigate symptoms, treatment needs, care coordination, and complex clinical situations.
You will conduct comprehensive assessments, monitor member health, and collaborate closely with nurses, medical directors, providers, and care teams.
The role emphasizes longitudinal oncology care and symptom management rather than hospital admission or discharge coordination.
You will also help address social needs, strengthen connections across the care network, and support members through survivorship or end-of-life care.

- Conduct comprehensive clinical assessments and provide ongoing triage, education, symptom management, and clinical support to members through phone and email.
- Support members across the full oncology care continuum, from screening and active treatment through survivorship and end-of-life care.
- Monitor member health, identify emerging clinical needs, and respond promptly to urgent issues and escalations.
- Provide care coordination support, including nutritional resources, provider referrals, discharge planning, and assistance navigating appropriate services.
- Build trusting relationships with members and their care networks through empathy, active listening, clear communication, and consistent follow-up.
- Collaborate with Nurse Team Leads, Medical Directors, oncology providers, primary care providers, payers, and other members of the care team to ensure coordinated, high-quality care.
- Participate in case conferences, provide updates on member progress, and contribute to targeted and individualized support plans.
- Partner with non-clinical team members to address social determinants of health, including transportation, food access, and other practical barriers to care.
- Provide clinical consultation and support for urgent escalations while following established clinical protocols and best practices.
- Maintain accurate, timely documentation and follow care team policies, procedures, and quality standards.
- Become proficient with internal systems, technology, tools, and partner workflows while managing a target of at least 20 member calls per day.
- Contribute to additional projects and initiatives aligned with department priorities and business needs.
Requirements:
- Active, unrestricted compact Registered Nurse (RN) license is required.
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required; BSN preferred.
- Compact and/or single-state licensure in New York and/or Illinois is preferred.
- At least 5 years of professional nursing experience, including a minimum of 3 years of solid tumor oncology nursing experience.
- Oncology-related certification such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) is required, or must be obtained within 2 years of hire.
- Strong clinical assessment, triage, symptom management, care coordination, and escalation-management skills.
- Demonstrated ability to build rapport with members and communicate effectively with patients, caregivers, providers, and multidisciplinary teams.
- Highly organized and able to manage multiple priorities, detailed documentation, and frequent member interactions in a fast-paced remote environment.
- Member-centered mindset with strong empathy, active listening, and commitment to delivering a supportive care experience.
- Action-oriented approach with the ability to recognize urgent needs, prioritize effectively, and take appropriate clinical action.
- Adaptability and comfort working in dynamic environments with evolving processes, technology, and priorities.
- Fluency in Spanish, both written and verbal, is preferred.
- Willingness to collaborate across clinical and non-clinical functions and contribute to a team-oriented care model.
