Position Summary
The Pediatric In-Home Nurse provides skilled nursing services to a pediatric client in their home. This role focuses on delivering individualized care, monitoring health status, providing education and support to family members, and collaborating with healthcare providers to ensure the highest quality of care. The nurse will work closely with one client, allowing for meaningful connections and continuity of care.
Responsibilities and Duties Include (but are not limited to):
- Provide skilled one-on-one nursing care to a pediatric client in the home
- Assess patient needs using the nursing process on an ongoing basis
- Observe, document, and report changes in condition to physicians and appropriate healthcare professionals
- Communicate effectively with family members and the healthcare team
- Provide education and training to clients and family members
- Administer and monitor medications as ordered
- Assess vital signs, skin integrity, nutritional status, and mobility
- Complete wound care and dressing changes as needed
- Assist with catheter care and other nursing procedures
- Perform blood draws and other skilled nursing tasks as ordered
- Monitor medication compliance and overall health status
- Maintain accurate and timely documentation
- Delegate tasks in accordance with the South Dakota Nurse Practice Act
- Promote client independence, dignity, and quality of life

- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in South Dakota
- Two years of nursing experience preferred
- Strong assessment, communication, and organizational skills
- Must be able to physically perform the duties of the position
- Must be able to read, write, and speak English
- Valid driver's license, reliable transportation, and proof of auto insurance required
- Proof of eligibility for employment in the United States
Physical Requirements:
- Walking, standing, bending, and lifting up to 50 lbs
- Ability to perform direct patient care activities
- Extended periods of documentation and communication
- Ability to work within a home care environment
