Position: Clinical Manager

Supervisor: Director of Clinical Services

Revision Date: 6/30/2026

FLSA Status: Exempt

Schedule: Full-time, Days

The Clinical Manager is a licensed Registered Nurse or qualified Therapist who supports the Director of Clinical Services/Administrator in the oversight of home health and hospice clinical operations in accordance with Pennsylvania Department of Health regulations, Medicare Conditions of Participation, and agency policies.

This role ensures the delivery of safe, effective, and compliant patient care; provides supervision of clinical staff; coordinates interdisciplinary services; and supports Quality Assurance and Performance Improvement (QAPI) activities. The Clinical Manager or a qualified designee is available during all operating hours, as required by regulation.

Key Responsibilities

1. Clinical Leadership & Program Oversight

  • Ensure patient care is deliveredin accordance withphysician/authorized practitioner orders and regulatory requirements
  • Oversee development, implementation, and review of individualized plans of care in compliance withstate and federal regulationsaccording to scope of practice
  • Monitor quality, timeliness, and appropriateness of servicesfor team
  • Review clinical records to ensurestate and federal regulatorycompliance. Ensure documentation supports eligibility, medical necessity, and reimbursement requirements
  • Investigate and report patient care incidentsperagency policy and Pennsylvania reporting requirements
  • Serve as a resource forassignedstaff

2. Staff Supervision & Development

  • Provide supervision ofassigned staff
  • Ensure competency validation and ongoing performance evaluation ofassignedstaff
  • Oversee recruitment, training, and ongoing educationin accordance withstate and regulatory guidelines.
  • Provide mentorship, coaching, and professional development opportunities
  • Conduct supervisory visits and documentation review as appropriate

3. Compliance & Documentation

  • Ensuretherapydocumentation meets regulatory, agency, and payer requirements
  • Participate in clinical record review to ensure compliance withstate and federal regulations, agencypoliciesand accrediting body standards
  • Support audit readiness and survey preparedness

4. Quality Assurance & Performance Improvement (QAPI)

  • Activelyparticipatein and support the agency’s QAPI program
  • Assistwith audits, performance improvement projects, and root cause analyses
  • Collaborate with the leadership team toidentifyopportunities for improvement and lead performance improvement projects
  • Ensure corrective actions are implemented and sustained

5. Collaboration & Leadership

  • Serve as a key member of the leadership team
  • Collaborate withleadership teamto align operational and clinical goals
  • Collaborate with interdisciplinary teams, including intake, RNs, therapists, and support staff, to optimize patient outcomes. 
  • Lead orparticipateinstaff meetings andcommittees (QAPI, Infection Control, Safety, etc.)
  • Promote a culture of safety, compliance, and patient-centered care

  • Knowledge of Home Health and Hospice state and federal regulations and Conditions of Participation.

  • Ability to ensure regulatory compliance and clinical quality
  • Ability to collaborate effectively with interdisciplinary teams.

  • Requires walking, standing, lifting (up to 25lbs), bending, and driving.

Education and Other Requirements

  • Graduate of an accredited school of nursing or therapy program
  • Current, active Pennsylvania license in applicable discipline
  • Minimum of 3–5 years of clinical experience; home health or hospice experiencerequired
  • At least one (1) year of supervisory or leadership experience preferred
  • A valid driver’s license, reliable transportation, proof of auto insurance with acceptable driving history isrequired
