Has an immediate opening for

Nurse Practitioner

(Permanent opportunity)

START DATE: Immediate

HOURS: up to 10 hours, shifts include rotational day, evening and night shifts as required

SALARY: $41.69 to $69.13 per hour

Purpose

Bring your knowledge and experience to St. Lawrence Youth Association, (SLYA) to work with young persons in conflict with the law. Reporting to the Director of Residential Services, the PHCNP is responsible for the overall health and well-being of the client in the residential care of the Association. Promoting health protection and quality care delivery, the PHCNP is self-regulated and governed by the College of Nurses in Ontario. The role of the PHCNP consists of direct clinical practice and incorporates the use of holistic perspective, formation of therapeutic partnership with the client, expert critical thinking skills, use of reflective practice, and evidence-based practice to guide care. Additionally, the PHCNP role includes expert coaching and guidance of clients, consultation, research, leadership, collaboration and ethical decision-making.

The PHCNP’s practice reflects substantial autonomy and independence with a high level of accountability. The PHCNP supports the goals and objectives of the St. Lawrence Youth Association and aligns with its mission, vision, values, and principles.

Mandatory requirements:

  • Registered with the Ontario College of Nurses at the extended class level
  • Clinical experience in a variety of health care settings (pediatric, women’s health, mental health)
  • Work independently
  • Valid Province of Ontario ‘G’ Driver’s License
  • Criminal reference check with Broad Record Check report

Duties/Responsibilities:

  • Performs comprehensive health assessments, physical and psychosocial assessment of the client’s status and develops and implements a plan of care to address needs using sound clinical, judgment, diagnostic and therapeutic skills
  • Consults with other health care professionals when the client's condition requires care beyond the scope practice. Experience in counseling and patient teaching
  • Ability to use multiple assessment strategies and critical thinking skills to guide decision-making in complicated, unpredictable and volatile situations
  • Prescribes medications, radiography, diagnostic tests, drawing blood and reading ECG in accordance with the Standards of Practice
  • Implements strategies to promote health and prevent illness
  • Refers clients to other providers or programs according to needs
  • Implements best practice guidelines to maintain quality of care of clients (e.g. writing policies)
  • Communicate health care needs as required to staff
  • Mentor to other learners (nursing and medical)
  • Responsible for healthcare inventory and equipment maintenance (e.g. Sterilization, vaccines etc.)
  • Maintain confidential medical records in compliance with standards
  • Provide consultative services to Dietary and Health & Safety of Association

EDUCATION: Proof of Education Degree in relevant field of study and/or 3 years experience
