Position Overview

The Behavioral Health Care Manager is a core member of the Collaborative Care Management team, working closely with patients’ medical providers, a consulting Psychiatric Consultant, and the broader primary care team.

This position supports and coordinates the behavioral and physical healthcare of patients on an assigned caseload. The Behavioral Health Care Manager provides patient engagement, behavioral health screening, brief evidence-based interventions, treatment monitoring, care coordination, and follow-up support.

Candidates may be considered for opportunities at our Chicago Heights, South Holland, Joliet, or Carpentersville health centers.

Key ResponsibilitiesPatient Assessment and Engagement
  • Screen and assess patients for common mental health and substance use disorders.

  • Engage patients in treatment and facilitate ongoing follow-up care.

  • Educate patients about behavioral health conditions, substance use concerns, and available treatment options.

  • Build effective and professional therapeutic relationships with patients.

Collaborative Care Coordination
  • Coordinate behavioral and physical healthcare with medical providers, the Psychiatric Consultant, and other treating professionals.

  • Participate in regularly scheduled caseload consultations with the Psychiatric Consultant.

  • Communicate treatment recommendations to patients’ medical providers and other members of the care team.

  • Help facilitate treatment-plan changes for patients who are not improving as expected.

  • Connect patients with more intensive or specialized behavioral health services when clinically appropriate.

Brief Behavioral Interventions
  • Provide brief, evidence-based interventions using techniques such as:

    • Motivational interviewing

    • Behavioral activation

    • Problem-solving treatment

    • Other appropriate evidence-based approaches

  • Support patients in developing practical strategies for managing symptoms and reaching treatment goals.

  • Develop relapse-prevention and self-management plans for patients preparing to complete the program.

Medication and Outcome Monitoring
  • Support psychotropic medication management as prescribed by medical providers.

  • Monitor treatment adherence, effectiveness, side effects, complications, and changes in clinical symptoms.

  • Track treatment response through in-person, telephone, and virtual contact.

  • Re-engage patients who have missed follow-up appointments or become disconnected from care.

Referrals and Community Resources
  • Facilitate internal and external referrals for clinically indicated services.

  • Connect patients with specialty behavioral healthcare, substance use treatment, housing assistance, vocational rehabilitation, and other community-based resources.

Documentation and Registry Management
  • Track patient follow-up and clinical outcomes using the Collaborative Care patient registry.

  • Document in-person, telephone, and virtual patient encounters.

  • Maintain timely and accurate documentation in the Electronic Health Record and other required systems.

  • Share relevant patient progress and treatment recommendations with authorized members of the care team.

Required Licensure or Education

Candidates must hold one of the following:

  • Licensed Clinical Social Worker, Licensed Social Worker, Licensed Clinical Professional Counselor, Licensed Professional Counselor, Licensed Marriage and Family Therapist, Registered Nurse; Bachelor of Science in Nursing preferred, Nurse Practitioner, Licensed Clinical Psychologist

Preferred Skills and Experience
  • Experience screening, assessing, or treating common mental health or substance use disorders.

  • Ability to work collaboratively within an interdisciplinary healthcare team.

  • Strong patient-engagement and therapeutic communication skills.

  • Experience with behavioral health assessments and treatment planning.

  • Knowledge of evidence-based counseling and brief behavioral interventions.

  • Basic knowledge of psychopharmacology within the scope of the license held.

  • Comfortable working with patients in person, by telephone, and virtually.

  • Experience in primary care, integrated care, or a Collaborative Care Model setting is a plus.

  • Strong organization, follow-up, communication, and documentation skills.
