Provides direct patient care under the supervision of the Licensed Physical Therapist; responsible and accountable to know and understand all OZH policies, vision, mission, values and standards.
Current valid driver’s license

Current valid driver’s license
- Vehicle liability insurance coverage
- Clear or acceptable motor vehicle record
- Initial MVR and annual MVR
