Case Manager in Brentwood, New York at RiseWell Community Services

RiseWell Community Services is seeking a full-time Case Manager to join our Residential Department. This is a hybrid position, and in the role, the Case Manager will be reporting directly to the program supervisor.

Job Function:
Provide case management services, housing supports, and coordination of services to people with psychiatric disabilities living in scattered apartment settings.

Qualifications:
High School Diploma and at least 1 year of experience in mental health. BA in Human Services and 1 year of experience preferred. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, and approval from the NYS Office of Mental Health.
