Vision Healthcare, LLC is looking for an experienced Clinical Supervisor & Field Assessment Nurse to help lead our growing clinical team. What You'll Do
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Follow-Up, Supervisory, and Discharge visits.
- Perform comprehensive patient assessments and develop individualized Plans of Care.
- Develop and manage CMS-485 Plans of Care and physician orders.
- Review clinical documentation for quality, accuracy, and regulatory compliance.
- Mentor and support RNs, LPNs, and other clinical staff.
- Conduct clinical competencies, field training, and new nurse orientation.
- Provide real-time clinical guidance and problem-solving support.
- Collaborate with physicians, case managers, caregivers, pharmacies, and interdisciplinary team members.
- Participate in Quality Assurance and Performance Improvement initiatives.
- Promote compliance with Medicare, Virginia DMAS, and agency policies.
- Serve as a clinical leader who helps strengthen our culture of excellence.
Qualifications
We're looking for an RN who is confident, clinically strong, and passionate about mentoring others.
Required
- Active Virginia RN license
- Minimum five years of recent Home Health experience
- Experience independently completing SOC, ROC, Recertification, Follow-Up, and Discharge visits
- Experience with OASIS documentation
- Experience developing CMS-485 Plans of Care
- Strong knowledge of Medicare Conditions of Participation and Virginia DMAS regulations
- Previous leadership, preceptor, or supervisory experience
- Excellent communication and critical-thinking skills
- Valid driver's license, reliable transportation, and auto insurance
