JOB SUMMARY

Reporting to the Health Care Administrator, the Director of Nursing (DON) plans, develops, and directs the clinical operations and services at Williamsburg Landing Health and Rehabilitation Center. Overseeing a 44-bed skilled nursing and long-term care unit within a Continuing Care Retirement Community (CCRC), the DON ensures the highest quality of clinical care is delivered in accordance with current federal, state, and local standards, guidelines, and regulations. The DON maintains rigorous compliance with CMS requirements and CARF-CCAC accreditation standards, establishes departmental performance goals, manages fiscal resources, leads team members, and champions a culture of Quality Assurance and Performance Improvement (QAPI).

ESSENTIAL JOB FUNCTIONS

  • Develops, maintains, and implements nursing service objectives, evidence-based standards of practice, and policy manuals in close collaboration with the Health Care Administrator and the Chief Clinical Officer.
  • Designs, implements, and monitors formal Plans of Correction for any identified past non-compliance or survey deficiencies. Actively involves floor nurses and Certified Nursing Assistants (CNAs) in actionable improvement plans to ensure frontline buy-in.
  • Under the direction of the Health Care Administrator, develops, conducts, and sustains a formalized, data-driven Quality Assurance Performance Improvement (QAPI) program.
  • Leads comprehensive internal investigations alongside the Health Care Administrator, clinical leaders, and Security/local law enforcement regarding any allegations of elder abuse, neglect, exploitation, or misappropriation of property; ensures mandatory reporting timelines are strictly met.
  • Leads weekly interdisciplinary quality meetings to analyze clinical risk indicators (including resident weights, hospital readmissions, wound care, behaviors, falls, psychotropic medication utilization, and pain management). Initiates formal QAPI action plans when negative trends are identified.
  • Represents clinical services in multi-disciplinary care planning meetings, collaborating seamlessly with Therapy, Activities, Social Work, Dietary, and physicians to maintain holistic, resident-centered care plans.
  • Coordinates and evaluates care provided by ancillary and contracted specialists (e.g., Wound Physicians, Psychiatric Nurse Practitioners, Podiatrists, Dentists); ensures all external consult documentation is promptly integrated into the Electronic Medical Record (EMR).
  • Collaborates monthly with the MDS (Minimum Data Set) Nurse Manager to review CASPER quality reports, ensuring timely, accurate data transmission and launching targeted quality improvement initiatives based on findings.
  • Oversees the collection, verification, and input of clinical quality metrics into corporate dashboards and monthly tracking spreadsheets; leads corporate clinical/quality review calls as required.
  • Conducts routine medical record audits to catch documentation deficiencies, ensuring precise clinical justification is present for skilled residents under the Patient Driven Payment Model (PDPM) and insurance guidelines to support accurate reimbursement.
  • Conducts daily clinical rounds on resident floors to evaluate nursing practices, verify adequate equipment/supply availability, assess appropriate staffing levels, and directly gauge resident and family satisfaction.
  • Oversees the recruitment, interviewing, and selection of clinical staff. Partners with Nurse Managers, Shift Supervisors, and the Nurse Educator to ensure a robust onboarding, mentorship, and competency-based training process.
  • Maintains real-time accuracy of the clinical department’s position control document to ensure vacancies are accurately advertised and budgeted hours match operational needs.
  • Conducts monthly nursing team communication meetings. Oversees the timely completion of annual performance evaluations and holds clinical staff accountable to performance standards through consistent, fair, and timely corrective action processes.
  • Tracks employee turnover metrics and shares corporate responsibility for retention goals. Proactively reviews feedback from employee engagement platforms (e.g., We Care Connect) and Stay Interviews to co-investigate and implement culture-building initiatives.
  • Supervises the nursing department’s after-hours triage support for employee injuries, ensuring all workers' compensation incidents are thoroughly documented, treated, and submitted to Talent Management within required windows.
  • Participates actively in the development of the annual departmental operating and capital budgets; monitors labor costs, overtime, and clinical supply utilization to ensure fiscal responsibility without compromising care.
  • Maintains highly visible, positive interactions with residents, families, visitors, and campus staff. Monitors resident satisfaction surveys to continuously improve the perception and delivery of service quality.
  • Assumes "on-call" administrative duty in a regular leadership rotation and establishes the nursing administration on-call schedule to guarantee 24/7 clinical oversight.
  • Directs continuing education plans for nurse leaders, fostering professional development and encouraging advanced clinical certifications.
  • Participates in community committees and performs other duties as assigned by the Health Care Administrator.

QUALIFICATIONS

  • Bachelor of Science in Nursing (BSN), preferred; Associate Degree in Nursing (ADN) with significant equivalent leadership experience considered.
  • Registered Nurse (RN) with a current, unencumbered license to practice in the Commonwealth of Virginia (or compact state).
  • Minimum of 3–5 years of progressive nursing leadership experience (e.g., as a Director of Nursing, Assistant DON, or Resident Care Director) within a skilled nursing facility, sub-acute rehabilitation center, or CCRC environment.
  • Director of Nursing Services-Certified (DNS-CT) or Certified Director of Nursing (CDONA) highly desirable.
  • Current CPR / BLS Certification.

KNOWLEDGE, SKILLS, AND ABILITIES

  • Deep working knowledge of CMS state and federal regulations, PDPM reimbursement structures, and QAPI frameworks
  • Prior experience managing clinical operations through a modern Electronic Medical Record (EMR) platform (e.g., MyUnity, PointClickCare)
  • Current knowledge of senior living industry trends and the ability to interpret and apply these trends to current plans and projects at Williamsburg Landing
  • Strong supervisory skills with the ability to motivate, train, and evaluate staff; skilled in resolving workplace conflicts and promoting teamwork
  • Excellent organizational and administrative skills and the ability to handle multiple priorities simultaneously in a fast-paced environment
  • Ability to maintain professional working relationships and work compassionately and professionally with employees, residents, prospective residents, family members, visitors, and the general public
  • Ability to work as a team or independently and be self-motivated and goal oriented with shared accountability for results and confidentiality
  • Ability to take initiative to solve problems on behalf of residents/prospective residents and employees and to exercise sound professional judgment appropriately in emergency situations
  • Exceptional interpersonal and verbal and written skills, with the ability to productively communicate with residents, families, and interdisciplinary teams.
  • Demonstrated experience in managing a budget
  • Proficiency with Google Suite, Microsoft Word, and Microsoft Excel; demonstrated experience with electronic medical records and other systems instrumental to nursing home administration
  • Ability to lift up to 25 lbs. occasionally
  • Ability to stand and walk throughout the shift, as well as ability to move about the community to attend meetings, visit team members, and participate in or present at activities as needed.
