Are you an experienced and highly autonomous Nurse Practitioner (NP) or Physician Assistant (PA) looking for a unique opportunity to provide meaningful, high-level clinical support to seniors while enjoying an exceptional work-life balance? Seniority Healthcare is seeking a dedicated Advanced Practice Clinician to serve in a combined On-Call and Provider of the Day role supporting our geriatric population and clinical teams.
This fully remote position follows a 7 days on / 7 days off schedule, offering flexibility and extended time off while allowing you to make a substantial impact across our organization.
About the Role
This unique clinical support role combines Provider of the Day responsibilities with after-hours On-Call coverage for a high-risk geriatric population.
During your scheduled week on, you will serve as the central clinical support provider for the organization, assisting field providers, addressing urgent patient concerns, conducting telemedicine visits, managing remote clinical needs, and ensuring continuity of care across the practice.
Telemedicine care is a core component of this role and includes both scheduled and urgent same-day patient evaluations. Unlike a traditional outpatient role, this position does not include management of a primary patient panel or routine clinic responsibilities.
This role is ideal for a confident clinician who enjoys problem-solving, acute clinical decision-making, virtual care delivery, and collaborating with providers and staff in a fast-paced environment.
Provider of the Day ResponsibilitiesAs Provider of the Day, you will provide real-time clinical support to the organization during daytime business hours by:
Conducting telemedicine visits for acute and urgent patient concerns
Managing same-day virtual evaluations to help prevent unnecessary ER visits and hospitalizations
Reviewing urgent clinical issues, laboratory and diagnostic results, and coordinating appropriate follow-up care
Supporting providers and nursing staff with acute care questions, treatment recommendations, and escalated triage concerns
Managing medication refill requests and other time-sensitive clinical needs
Assisting with transitions of care and hospital discharge follow-up
Facilitating timely patient communication and overall care coordination
Supporting clinical workflow needs across the practice while collaborating closely with providers, nurses, and operational staff to ensure high-quality patient care
During your scheduled week on, you will also provide after-hours clinical coverage for our patient population.
On-Call ScheduleWeekdays: 4:00 PM – 8:00 AM
Weekends & Holidays: 24-hour coverage
All patient calls are first screened through our answering service, which triages and forwards clinically appropriate concerns to the on-call provider.
On-Call Duties Include:Conducting after-hours telemedicine visits as clinically appropriate
Addressing urgent patient concerns and symptom management remotely
Providing clinical guidance and treatment recommendations
Determining appropriate escalation of care, including emergency evaluation when necessary
Managing time-sensitive medication concerns and refill requests
Supporting patients, families, facilities, and staff during urgent situations
Coordinating urgent follow-up care and communication with the clinical team
Documenting all after-hours encounters appropriately within the EHR
Completion of an accredited Nurse Practitioner or Physician Assistant program
Active and unrestricted NP or PA license
Must be licensed in Pennsylvania, New York, New Jersey, and Delaware
Willingness to obtain licensure in additional states as the organization expands (licensure costs reimbursed by the company)
Experience in geriatrics, family medicine, internal medicine, or emergency medicine preferred
Prior telemedicine experience preferred
Strong clinical judgment and ability to work independently
Excellent communication, organization, and problem-solving skills
Comfortable managing complex geriatric patients remotely and collaboratively
Ability to thrive in a fast-paced, team-oriented environment
