Overview:

Under the direction of the Chief Medical Officer and the oversight of the supervising/assigned Physician, the Nurse Practitioner provides comprehensive care to Trillium Health patients. The scope of practice includes primary and/or specialty care delivery (HIV/HepC, PEP/PrEP, Transgender/LGBT Health), education, health promotion, and collaboration with patients, families, and all Trillium Health staff. The ability to work as a member of a team is critical.
Key Responsibilities:
Clinical Requirements
- Develop an acceptable practice agreement with a collaborating physician.
- If Nurse Practitioner has more than3600 hoursof qualifying NP experience they can opt for the following:
- Practicein accordance witha written practice agreement with a collaborating physician or practice and have collaborative relationships with a qualified physician or a New York State Health Department licensed healthcare facility.
- Submit proof of the practice agreement withan appropriate physician, licensed to practice in New York State, to the State Education Department within90 daysof employment.
- Utilize the ANA Scope of Practice forThe PrimaryHealth Care Nurse Practitioner and Standards of Practice for the Primary Health Care Nurse Practitioner statements reflecting the scope of practice.
- Maintainappropriate credentialsto practice at Trillium Health.
Patients
- Performhistoriesandphysicals,routineand urgent office visits.
- Review laboratory and test results and perform office procedures as experience and credentialing allow.
- Provide health teaching to patients and families andparticipatein educational programming.
- Promote health including nutrition, stress management, sexuality concerns, contraception, smoking cessation, and substance use support.
- Participate in case management and team-based care.
- Supervise clinical interventions provided by registered nurse staff in partnership with physician providers.
- Engage in continuing education in primary care, HIV/AIDS care, and related topics.
- Perform peer reviews and chart reviews per agencypolicies.

Required Key Skills and Qualifications:
- Minimum 3 years’ experience as a board-certified Nurse Practitioner.
- M.S. degree as a nurse practitioner from a National League for Nursing (NLN) accredited school of nursing.
- Current licensure in New York State as a Registered Professional Nurse and current certification as a Nurse Practitioner.
- National certification (ANA or equivalent; adult or family preferred).
- Experience in community-based healthcare preferred.
- EMR familiarity preferred.
- Strong teamwork, interpersonal, communication, and computer skillsrequired.
Physical Requirements
While performing the duties of this job, the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms; talk; and hear. Occasionally the employee must stoop, bend, and lift or move up to 25 lbs. Vision requirements include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
