Care Coordinator - Youth CCS in Milwaukee, Wisconsin at Wisconsin Community Services

This is a community-based, mobile position where you’ll meet people where they are emotionally, physically, and within their communities. You’ll work alongside individuals, families, providers, and multidisciplinary teams to ensure each person receives coordinated, person-centered support.

Pay range begins at $52,000

What You’ll Do

• Build meaningful relationships by meeting individuals where they are and creating trusting, respectful, and supportive connections.
• Coordinate individualized care by partnering with individuals, families, natural supports, service providers, and recovery teams to develop, implement, and update personalized recovery plans.
• Connect people to resources by identifying community services and completing and sending referrals for needed supports.
• Collaborate with providers including therapists, healthcare professionals, case managers, and other community partners to promote coordinated, high-quality care.
• Support recovery goals by monitoring progress toward self-identified goals and providing supportive coaching, psychosocial rehabilitation, and symptom-management support.
• Stay connected through regular in-person visits, phone calls, and follow-ups.
• Help remove barriers by ensuring individuals have access to transportation for program-related appointments.
• Advocate for individuals by helping them access timely and appropriate services while empowering them to recognize and build on their strengths.
• Support individuals through challenges by responding to crises and setbacks with empathy, professionalism, and a trauma-informed approach.
• Document services accurately by maintaining timely and thorough documentation in accordance with agency, county, regulatory, and funding-source requirements.
• Stay current and compliant by completing all required county and agency trainings and following agency policies, procedures, Code of Conduct, and Core Values.

What We’re Looking For

Required Qualifications

• Bachelor’s degree in Social Work or a related field.
• One year of experience working with individuals experiencing mental health, substance use, and/or co-occurring needs is preferred.
• Ability to meet all WCS and contractor requirements, including criminal and caregiver background checks, driver’s record review, and drug screening.
• Valid driver’s license, reliable automobile, and sufficient insurance to meet agency requirements.

Preferred Qualifications

• Master’s degree in Social Work or a related field.
• Bilingual abilities.
• Experience working with Milwaukee County Behavioral Health Services.

• The ability to communicate clearly and provide calm, compassionate support during times of crisis.
• Experience with assessment, interviewing, recovery-oriented approaches, and harm reduction models.
• A passion for mental health advocacy and person-centered care.
• Strong verbal and written communication skills and the ability to work effectively with diverse individuals, families, coworkers, supervisors, and community partners.
• The ability to work independently while also contributing to a respectful, collaborative team environment.
• Excellent organization, time management, and multitasking skills, with the ability to meet deadlines and adapt to changing priorities.
• Strong critical-thinking and solution-oriented skills.
• Proficiency with Microsoft 365, email, and internet applications.
• Experience with documentation, electronic health records, and maintaining accurate and timely records.
• Cultural sensitivity and awareness of ethnic, gender, and disability-related issues.
• A commitment to conducting yourself ethically and professionally in every interaction.
• A commitment to the agency’s mission, values, Code of Conduct, policies, procedures, and funding-source requirements.

What to Expect

This is a mobile, community-based position averaging 40 hours per week. Your work may take you into individuals’ homes, community locations, offices, and meetings throughout Milwaukee County.

You should be comfortable:

• Traveling throughout Milwaukee County.
• Driving and transporting individuals when needed.
• Navigating stairs and working in a variety of community settings.
• Maintaining a flexible schedule based on the needs of the people we serve and the team.
• Working in both office and community environments.
• Responding to changing needs and priorities throughout the day.
