As an RN on the ACT Team, you will provide comprehensive nursing care, coordinate health services, and support individuals living with severe and persistent mental illness and co-occurring conditions. Position SummaryThe Registered Nurse (RN) – Assertive Community Treatment (ACT) Team provides comprehensive nursing care and clinical support to individuals with serious and persistent mental illness and co-occurring disorders in a community-based setting.
Working as part of a multidisciplinary ACT Team, the RN is responsible for medication management, health assessments, care coordination, health education, crisis intervention, and treatment planning. The RN plays a critical role in supporting recovery-oriented services that promote wellness, stability, independence, and successful community living.
This position provides care in homes, community settings, and other locations where clients live, work, and receive services.
Essential ResponsibilitiesClinical Nursing Care- Conduct comprehensive psychiatric, clinical, and physical health assessments.
- Monitor and evaluate clients' physical and behavioral health status.
- Administer and manage medications and prescribed treatments.
- Complete vital signs, metabolic screenings, injections, and other ordered nursing interventions.
- Monitor medication effectiveness, side effects, and treatment outcomes.
- Maintain accurate medication records and documentation.
- Ensure proper medication storage, inventory management, and regulatory compliance.
- Provide nursing services within homes and community settings.
- Assist clients with activities that support independent living, including:
- Medication self-management
- Activities of Daily Living (ADL) skill development
- Home management and wellness education
- Health promotion and disease prevention
- Promote client independence and recovery through individualized interventions.
- Educate clients on:
- Mental health symptoms and treatment
- Medication purpose and adherence
- Chronic disease management
- Wellness and preventive healthcare practices
- Provide support, education, and consultation to family members and natural supports.
- Empower clients to actively participate in their treatment and recovery process.
- Coordinate healthcare services with physicians, specialists, pharmacies, hospitals, and community providers.
- Ensure clients attend medical appointments and receive recommended follow-up care.
- Facilitate referrals and connections to healthcare, housing, employment, and community resources.
- Build collaborative relationships with external providers and service partners.
- Respond to psychiatric, medical, and behavioral health crises.
- Coordinate emergency services and interventions when necessary.
- Provide clinical support during urgent and high-risk situations.
- Participate in on-call responsibilities as required.
- Participate in interdisciplinary treatment planning meetings and case conferences.
- Contribute nursing assessments and recommendations to individualized treatment plans.
- Communicate significant treatment updates and health concerns to team members and providers.
- Collaborate closely with psychiatrists, therapists, case managers, peer specialists, and other ACT Team members.
- Complete accurate, timely, and comprehensive clinical documentation.
- Maintain compliance with:
- Nursing standards of practice
- Medication administration regulations
- Licensing requirements
- Agency policies and procedures
- HIPAA and confidentiality standards
- Support infection control and communicable disease prevention activities.
- Provide consultation and clinical guidance to team members.
- Train and support unlicensed staff regarding medication administration, health monitoring, and client care procedures.
- Promote best practices in behavioral health nursing and recovery-oriented care.
- Advocate for client rights, access to services, and quality healthcare.
- Assist clients in navigating complex healthcare and social service systems.
- Support community integration and self-determination.
- Current and unrestricted Registered Nurse (RN) license in the State of Delaware.
- Graduate of an accredited School of Nursing.
- CPR Certification.
- Successful completion of a pre-employment Tuberculin Skin Test (TST).
- Ability to obtain and maintain Delaware Department of Correction security clearance.
- Successful completion of all required background investigations.
- Valid driver's license, current vehicle registration, and reliable transportation.
- Bachelor of Science in Nursing (BSN).
- Minimum of one year of clinical nursing experience.
- Experience working in:
- Behavioral health
- Community mental health
- ACT Programs
- Substance use treatment
- Integrated healthcare settings
