ACT Team Psychiatric PrescriberPosition Summary

The ACT Team Psychiatric Prescriber provides clinical leadership and psychiatric services within a multidisciplinary Assertive Community Treatment (ACT) team. This position is responsible for the psychiatric assessment, diagnosis, treatment, and ongoing care of individuals experiencing severe and persistent mental illness, substance use disorders, and co-occurring medical, housing, employment, and daily living challenges.

The Psychiatric Prescriber collaborates with team members to develop and oversee individualized, recovery-oriented treatment plans that are person-centered and focused on each client's strengths, needs, preferences, and goals. The position promotes evidence-based, trauma-informed care while ensuring continuity of services, coordinating medical and behavioral healthcare, responding to crises, and maintaining accurate clinical documentation.

Essential Duties and ResponsibilitiesClinical Services
  • Conduct comprehensive psychiatric evaluations, assessments, and diagnostic reviews in accordance with DSM criteria.
  • Diagnose behavioral health conditions and establish appropriate treatment recommendations.
  • Prescribe, monitor, and adjust psychotropic medications and related treatments based on clinical need.
  • Perform medication management services and periodic medication reviews.
  • Provide face-to-face psychiatric services in office, community, hospital, and home-based settings as appropriate.
  • Monitor medical conditions and review non-psychiatric medications that may impact treatment outcomes.
  • Utilize Medication-Assisted Treatment (MAT) when clinically appropriate for individuals with substance use disorders.
  • Participate in involuntary psychotropic medication hearings as requested.
  • Provide crisis intervention and emergency psychiatric consultation as needed.
  • Communicate and collaborate with hospitals and other treatment providers during client admissions, transfers, and discharges.
Leadership and Team Collaboration
  • Provide clinical leadership and guidance to the multidisciplinary ACT Team.
  • Collaborate with the Team Leader in the development, implementation, evaluation, and revision of recovery plans.
  • Participate actively in daily team meetings, treatment planning, and care coordination activities.
  • Provide consultation, psychiatric education, and clinical support to non-medical staff.
  • Foster a culture of hope, recovery, wellness, and trauma-informed practice.
  • Model professional behavior and a commitment to the mission and values of Conexio Care.
  • Maintain professional boundaries and ethical therapeutic relationships.
Documentation and Compliance
  • Complete all clinical documentation accurately and timely within the electronic health record (EHR).
  • Prepare reports and psychiatric documentation as required.
  • Maintain compliance with agency policies and procedures as well as applicable federal, state, and regulatory requirements.
  • Protect client confidentiality and comply with HIPAA standards at all times.
  • Participate in required trainings to maintain clinical competence and stay current with evidence-based practices.
Care Coordination
  • Coordinate psychiatric care with primary care providers, medical specialists, hospitals, and community resources.
  • Maintain collaborative relationships with clients, families, providers, and community partners.
  • Support client autonomy, dignity, rights, and self-determination through nonjudgmental and culturally responsive care.
Additional Duties
  • Participate in on-call coverage as assigned.
  • Attend required meetings, trainings, and supervision sessions.
  • Perform other duties as assigned to support effective program operations.
Minimum QualificationsPhysician Qualifications
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
  • Successful completion of an approved psychiatry residency program.
  • Board Certified or Board Eligible in Psychiatry by a nationally recognized certifying body.
  • Current unrestricted license to practice medicine in the State of Delaware.
  • Active Drug Enforcement Administration (DEA) registration.

OR

Psychiatric Nurse Practitioner Qualifications
  • Graduate degree in advanced practice nursing from an accredited institution.
  • Current Delaware licensure and certification as a Psychiatric Mental Health Nurse Practitioner.
  • Authorized under Delaware law to diagnose mental health disorders and prescribe psychotropic medications.
  • Practices under the supervision of a Delaware-licensed psychiatrist and maintains required supervision documentation, including a minimum of two (2) hours of supervision per week specific to individuals served.
  • Minimum of five (5) years of experience as a Nurse Practitioner.
Preferred Qualifications
  • Minimum of two (2) years of experience practicing as a psychiatric prescriber within a multidisciplinary treatment team.
  • Experience providing services in community behavioral healthcare settings.
  • Strong knowledge of psychiatric assessment, diagnosis, treatment planning, and psychopharmacology.
Additional Requirements
  • Maintain a satisfactory criminal background investigation as required by agency policy and applicable regulations.
  • Possess a valid driver's license and maintain an acceptable driving record.
