Care Coordinator – Entry Level in Portage, Michigan at Visiting Angels of Southwest Michigan

Visiting Angels is looking for an Support Professional to support our caregivers and clients. This position combines caregiver leadership, training, office support, and hands-on client care.

• Support, coach, and train Angel Caregivers
• Assist with client Care Plans and caregiver training
• Help resolve caregiver and client concerns
• Help onboard and orient new caregivers
• Provide in-home client care as needed by assisting clients with personal care, meals, medication reminders, light housekeeping, errands, transportation, companionship, and mobility
• Complete required documentation
• Communicate with caregivers, clients, families, and the Support Team
• Assist Care Coordinators as needed
• Participate in evening and weekend on-call rotation as directed
• Assist with general office and team needs

• At least 18 years old
• Caregiving or home care experience preferred
• Strong communication and leadership skills
• Dependable, organized, and comfortable working independently
• Basic computer and Microsoft Office skills
• Compassionate and professional attitude
• Ability to lift at least 50 lbs.
• Ability to meet required background, health, training, and employment requirements
