Statement of Purpose

To provide direct services to patients and/or families in understanding and adapting to the realities imposed by illness, as well as intervention and plan of care to meet social/health needs in coordination with providers and multidisciplinary team utilizing available community resources. Collaborates with multidisciplinary team to provide services directed at restoring patients to optimal social and health adjustment.

Job Requirements

Masters degree in Social Work and VT license required. At least one year’s experience actively working with patients in the health care field. Computer experience required.
