Pay: Dependent on certification status

• Uncertified Job Title range starts at $17.50 - Uncertified Job Title will obtain certification within the first 120 days and pay will be increased
• Certified Job Title range starts at $18.75 - Certified Job Title pay is calculated based on years of certified experience

Schedule: Monday – Friday | 8:00am – 5:00pm

Job Summary

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Essential Functions

• Provides psychosocial rehabilitative services as described by the Medicaid manual
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley’s standards
• Meets productivity expectations as assigned by supervisor
• Distributes medications as necessary
• Transports clients to appointments as necessary

Required Qualifications

Education

• High School diploma or equivalent

Experience

• None

Licenses/Certificates

• Case Manager certification (post hire as needed)
• CPR certification (post hire as needed)
• Driving positions require a minimum age of 21 and a current driver’s license
