About This Position

Nurse Practitioner

Position Summary
- Assessment and treatment in a primary care setting with particular attention to chronic diseases. Using EMR to order and document appropriately. Using $4 formularies prescribe medications. Assist with health education. Co-mange care with appropriate specialists.
Education
- Master's of Science in Nursing - Required
Experience
- 2-3 Years - Preferred
License & Certification
- Professional License with State of Practice - Required
- Advanced Practice NP - Required
- Basic Life Support (BLS) - Required
- Advanced Cardiovascular Life Support (ACLS) - Required
Core Job Functions
- Order labs, imaging, diagnostic, medicines appropriately with focus on cost and first step of action, rather than most expensive first. Sends in prescriptions and refills in a timely manner.
- Review results of orders in a timely manner and uses results to plan care. Instructs patients on the results, plan of care and outcome expectations.
- Investigates thoroughly before requesting specialists. Able to provide referral with all tests, action taken to diagnose and questions regarding irregular outcomes. Willing to co-manage with specialists.
- Incorporates SDoH in to the diagnosis and treatment. Documents major barriers that could contribute to the health problems. Refers to Social Worker or others for Community Resources.
- Documents appropriately and timely. Researches and consults as needed. Includes the patient in decision making and holds them accountable for agreed outcomes.
