In this role, you'll play a vital part in providing community-based behavioral health services, care coordination, and supportive interventions to individuals and families, helping clients build stability, independence, and healthier lives. Essential Job Duties:

• Provide community-based behavioral health services and supportive interventions in accordance with established treatment plans.
• Engage clients and families in person-centered, recovery-oriented care while helping develop coping, self-management, and independent living skills.
• Coordinate referrals, appointments, and connections to community resources such as housing, transportation, and benefits.
• Provide outreach, follow-up, and care coordination in collaboration with internal and external providers.
• Recognize and respond appropriately to behavioral health crises using approved de-escalation and safety intervention techniques.
• Complete accurate and timely documentation while maintaining confidentiality and compliance with HIPAA, Medicaid, and organizational requirements.
• Participate in interdisciplinary team meetings, required training, and ongoing competency development.

Qualifications:

• High School Diploma or GED required.
• Ohio CDCA Certification, preferred.
• Ability to be insured on the commercial auto insurance policy based on driving record including active driver’s license and personal automobile insurance and reliable transportation.
• Ability to be credentialed within 90-days of onboarding.
