Responsibilities Perform in-home nursing assessments

Provide skilled care per physician orders (wound care, meds, injections, vitals)

Monitor patient condition and report changes

Educate patients and caregivers

Document care in EHR

Follow safety and infection control standards

Qualifications Active LPN/LVN license

2+ years nursing experience (home health preferred)

Strong assessment and communication skills

EHR proficiency

Valid driver’s license and reliable transportation
