NP or PA for Long-Term Care | M-F | Days | CMHCF in Bolivar, Missouri at Citizens Memorial

Citizens Memorial Health Care Facility (CMHCF) is hiring a full-time Nurse Practitioner or Physician Assistant to serve as the primary provider for residents in long-term nursing care. This is a weekday role and you will collaborate with our Geriatrician to provide exceptional care to the residents at CMHCF.

You will provide care for one facility, which means you will develop meaningful relationships with your patients. You learn who sleeps late, whose son calls the nurses' station every Tuesday, and who prefers visits first thing in the morning or after a nap in the afternoon.

CMHCF holds a Silver Quality Award from the American Health Care Association and the National Center for Assisted Living. Provider retention here runs well above regional norms, which is usually the most honest thing a health system can tell you about itself.

The Opportunity

• Full-time, Monday – Friday Days with flexibility.
• Call: shared APP rotation, roughly one weekday call per week and 1:5 weekends
• Provide care across for residential patients including: monthly regulatory and routine follow-ups, annual wellness visits, Medicare routine visits, acute change-in-condition management, non-skilled admissions, discharge visits, and chronic disease management
• Limited outreach coverage may be incorporated, up to one day per week
• Interdisciplinary support on site: facility administrator, DON, MDS coordinator, clinical pharmacist, social services, PT/OT, and full nursing staff
• EMR: Meditech Expanse.

Life in Bolivar

Bolivar is a town of about 11,000 with a university, a quaint downtown square and a commute measured in minutes. Springfield is 30 minutes away for airport, shopping, and anything Bolivar does not have. Table Rock, Bull Shoals, and Mark Twain National Forest are close enough for a weekend or an evening. Kansas City is two hours up highway 13. Housing costs a fraction of what it does in a metro, which changes the math on an APP salary considerably.
