Job Description Summary:
We are seeking a dedicated and organized professional to be responsible for on-going meetings and communications with High Schools Transition Coordinator, etc. for the purpose of planning, implementing, and overseeing Pre-Employment Transition Services (PreETS) for student participants in a one on one or group setting. Coordinating the start-up and resolution of PreETS to ensure information is disseminated to the proper High School Transition Team, Funded partner and student participants/guardians. Responsibilities would also include reviewing reports, providing/writing recommendations for next steps, and attending team meetings to review the results of Services. Accountable for delivering PreETS services; developing community PreETS worksites/partnerships for services where student participants can gain real life knowledge of employment tasks, expectations, etc. Responsible for tracking and entering data related to PreETS and the Summer Youth Program, as well as to include that of other Youth Services Deliverables. Monitoring Service Authorizations and the curriculum to ensure that requirements are met, during the period of the authorized services and deadlines are met. Serve as a backup Youth Service Specialist for Summer Youth Work Experience Program, as well as Coordinating the required activities in preparation for the Summer Youth Program to include assisting with reviewing reports and additional duties as assigned.
MINIMUM REQUIREMENTS:
EDUCATION, EXPERIENCE, AND LICENSING REQUIREMENTS:
Education: A bachelor's or associate degree in human services, secondary special education or a related area is preferred, or a high school diploma/GED is required.
Experience: Twelve months of experience working with individuals with disabilities in a vocational or educational setting is preferred. Additionally, six months of providing case management or coordinating communication and services with teams of individuals with disabilities is required.
Knowledge/Abilities: Basic knowledge of behavior modification techniques is preferred. Knowledge of professional communication principles. Proficient knowledge of Microsoft Office, Outlook, Excel, and Word.
Excellent communication skills, both verbal and written. Professional and personable presentation, at all times. Must be able to understand and effectively communicate the English language. Must have great organizational and time management skills. Must have dependable transportation. Must have great customer service skills. Must be able to lift to 25 pounds.
ESSENTIAL DUTIES:
Track and communicate to in-house Youth Services Team of new Referrals and Authorizations.
Coordinates and complete Pre-Employment Transition Services, community work experiences, provided by the Youth Services Department.
Respond to, communicate with, and update referral sources on the progress of services.
Communicate with and update the Youth Services Supervisor and Youth Services Manager, regarding scheduling issues and other impacts to service delivery.
Maintain ongoing communication with designated team members regarding student participants and services.
Prepare monthly progress reports and other relevant participant-related documents.
Schedules and chairs Staffings for completed services.
Maintains records on travel expenses, billable hours, and work schedules.
Maintains and safeguards records following privacy policies.
Other duties as assigned concerning Youth Service departmental needs.
PERIODICAL DUTIES:
Transport student participants as needed to staff meetings held at Goodwill, etc.
Provides direct service for an average of 20 hours per week.
OCCASIONAL DUTIES:
Temporarily assist other departments with in-house needs.
