Key Responsibilities:
- Review and process medication refill requests efficiently and accurately in accordance with established clinical protocols and regulatory requirements.
- Verify patient information, medication history, and current treatment plans to ensure appropriate refills.
- Communicate with healthcare providers to obtain necessary approvals or clarifications for refill requests.
- Provide patient education regarding medication usage, potential side effects, and adherence strategies.
- Document all refill activities thoroughly in the electronic health record system.
- Support patient follow-up and coordinate care transitions as needed.
- Stay informed about FQHC guidelines and ensure compliance with all relevant policies and procedures.
Skills and Qualifications:
- Valid RN or LVN license in the state of practice.
- Prior experience in outpatient or clinical pharmacy settings preferred.
- Strong knowledge of medication management, clinical protocols, and regulatory standards.
- Excellent communication and interpersonal skills.
- Ability to multitask, prioritize, and work efficiently in a fast-paced environment.
