Case Management Coordinator - Transition of Care Team

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Case Management Coordinator - Transition of Care Team based in the United States.

This full-time remote role supports members as they navigate healthcare services, benefits, appointments, and transitions in care.
You will use critical thinking, care-management resources, and a member-centered approach to identify needs and remove barriers to appropriate care.
The role combines case coordination, benefit navigation, health education, documentation, and collaboration with clinical and multidisciplinary teams.
You will help identify higher-risk situations and ensure members receive appropriate referrals, services, and follow-up.
Success requires strong communication, sound judgment, organization, and the ability to motivate members toward informed healthcare decisions.
You will work within established quality, regulatory, accreditation, and privacy standards while managing a high volume of member needs.
This is a structured remote environment with a standard 8:00 AM–5:00 PM schedule in the assigned market time zone.

Accountabilities
  • Evaluate referred members using care-management tools, available data, and benefit information to understand needs, eligibility, barriers, and available resources.
  • Review member benefit plans and identify appropriate internal and external programs, services, and support options.
  • Identify high-risk factors and service needs that may affect outcomes, escalating cases to clinical case management or crisis intervention when appropriate.
  • Coordinate assigned care-plan activities, monitor progress, and follow up to help ensure identified needs are addressed.
  • Collaborate with case managers, supervisors, Medical Directors, and other healthcare programs to overcome barriers and support appropriate care outcomes.
  • Participate in case conferences and contribute information for multidisciplinary review when additional expertise is needed.
  • Identify and escalate quality-of-care concerns through established processes and communication channels.
  • Use negotiation, influencing, and motivational interviewing techniques to help members access appropriate benefits and healthcare services.
  • Provide members with coaching, education, information, and practical support that encourages independent healthcare and healthy lifestyle decisions.
  • Help members participate actively and knowledgeably in healthcare decisions with their providers.
  • Apply case-management and quality-management processes consistently while maintaining compliance with regulatory, accreditation, privacy, and organizational requirements.
  • Maintain accurate, timely, and complete documentation of member interactions, care coordination activities, and outcomes.
  • Navigate multiple systems and applications simultaneously while communicating effectively through email, messaging platforms, calendars, and virtual meetings.
  • Protect confidential information by following secure-system, password, device, VPN, and HIPAA-related practices.
  • Resolve routine technical issues independently and appropriately engage IT support when more complex assistance is required.
Requirements
  • At least 2 years of experience in behavioral health or social services.
  • Bachelor's degree in behavioral health or human services, such as psychology, social work, marriage and family therapy, or counseling, or a non-licensed master's-level clinician degree in a related field; equivalent experience may also be considered.
  • At least 2 years of experience using Microsoft Office applications, including Word, Excel, and Outlook.
  • Case management and discharge-planning experience is preferred.
  • Previous managed-care experience is preferred.
  • Strong critical-thinking and problem-solving abilities, with sound judgment when assessing member needs and determining appropriate next steps.
  • Excellent communication and interpersonal skills, with the ability to explain healthcare benefits, services, and options clearly and empathetically.
  • Ability to use motivational interviewing, influencing, and negotiation techniques to encourage member engagement and support positive health behaviors.
  • Strong organization and time-management skills, with the ability to coordinate multiple activities while maintaining accurate documentation.
  • Comfortable working across multiple monitors, applications, secure systems, EHR portals, and digital communication tools.
  • Working knowledge of Microsoft 365 tools such as Teams, Outlook, Word, Excel, and PowerPoint, or comparable productivity platforms.
  • Ability to follow information-security practices, including VPN access, strong password management, screen locking, and identification of suspicious emails or links.
  • Ability to troubleshoot common technology issues independently and escalate unresolved problems appropriately.
  • Willingness to learn new technologies, workflows, and skills as the remote workplace evolves.
  • Availability to work 8:00 AM–5:00 PM in the assigned market time zone.
  • Ability to maintain a dedicated, quiet, secure, and private home workspace free from interruptions and compliant with applicable privacy requirements.
  • Ability to maintain a direct, hardwired internet connection to the modem/router within 7 feet of the computer, with minimum speeds of 25 Mbps download and 3 Mbps upload; Wi-Fi and satellite internet are not permitted.
  • Dependents must have separate care arrangements during working hours to ensure uninterrupted availability.

  • Base pay range of $21.10–$40.90 per hour, with actual compensation based on factors such as experience, education, geography, and other relevant considerations.
