Responsibilities and Duties: Working under the supervision of the MAPs (Medicaid Alternative Pathways) Specialist Supervisor, the MAPs Specialist will work closely on innovation projects including the Medicaid Alternative Pathways program, Technology Implementation, and other developing projects as become relevant. The MAPs Specialist will assist in the development and implementation of new service models, identifying the systems and quality oversight to assist in wider adoption and implementation.

The MAPs Specialist demonstrates a strong awareness of how trauma can impact individuals and communities and approaches their work with compassion, cultural humility, and a commitment to inclusivity.

Specific duties include:

  1. Assists in the accomplishment of corporate strategic planning that achieves a high impact and significant improvement in the organizational performance in the areas of Enabling Technology, Supported Employment, and Independence Coaching as part of the MAPs program;
  2. Supports a MAPs caseload that includes:
  3. Maintaining a caseload of 11 to 13 MAPs individuals;
  4. Establishing check-in/workdays with the individual;
  5. Conducting in-person meetings with the individual at least once per month (unless receiving virtual services only), with scheduling based on the individual’s availability whenever possible;
  6. Completing and maintaining service templates;
  7. Submitting templates for review;
  8. Working with MillarRich Accounts Receivable for billing compliance and needs;
  9. Working collaboratively with Circles of Support (COS), MillarRich departments, and DDA (Department of Disability and Aging) to accomplish goals and meet benchmarks for applicable MAPs services;
  10. Working with the individual served and any other relevant departments to ensure all required and relevant information for the individual’s planning meeting is prepared and/or submitted;
  11. Participating in scheduled team meetings, coordinating communication across all team members, and coordinating any emergency meetings, as applicable;
  12. Providing feedback and assisting with the development of all necessary information for the individual’s plan including skill acquisition programs, as applicable;
  13. Providing routine status reports and updates, as needed, to the MAPs Specialist Supervisor on caseload achievements or concerns;
  14. Assists in the development of audit and oversight activities related to new services and/or processes;
  15. Conducts quality and compliance reviews of caseload-specific programs;
  16. Participates in DDA/MCO (Managed Care Organization)/Statewide conversations regarding MAPs, Technology, and Independence Training;
  17. Completes Independence Coaching Training and maintains relevant certifications;
  18. Completes Supported Employment (SE) training and relevant certifications as identified as necessary;
  19. Maintains close collaboration with the Supported Employment (SE) department for technology and employment referrals that would be assumed within that department;
  20. Maintains close collaboration with the residential services program in Community and Residential Services (CRS) to collaborate and plan for post-graduation and any other benefits assistance that may apply to the individuals in the MAPs program, if needed.
  21. Ensures that any changes in key stakeholder requirements, agency policies and procedures, and/or agency/department expectations are effectively disseminated;
  22. Other duties may be assigned as circumstances warrant.
Requirements:

Knowledge, Skills, Abilities and Personal Characteristics:

  1. A bachelor’s degree or at least two years of experience in a related field required.
  2. Proficiency in Microsoft Office Suite.
  3. Ability to quickly learn new programs and perform accurate, timely data entry.
  4. Attention to detail is mandatory in this position.
  5. Excellent interpersonal and human relations skills.
  6. Excellent oral and written communication skills.
  7. Ability to maintain confidentiality.
  8. Ability to work autonomously and maintain a flexible schedule as needed, including occasional evenings and weekends.
  9. Ability to plan, organize and prioritize events in a professional work environment and manage stressful situations using strong problem-solving skills.
  10. Ability and comfort with advocating for the well-being of individuals supported.
  11. Positive, professional, and approachable demeanor.
  12. Operation of a motor vehicle is required which includes proof of a valid driver’s license, an ability to maintain automobile liability insurance as required by state law in Tennessee, and an ability to qualify as a driver under MillarRich, LLC insurance guidelines.
