OVERVIEW

Working in collaboration with the market’s Network Director, Employment team, and Care Coordination team, the Community Engagement Coordinator supports the planning, coordination, and execution of community engagement initiatives that align with Veterans Bridge Home’s mission and annual strategic goals. This role focuses on identifying, engaging, and building meaningful connections with Service Members, Veterans, and their Families (SMVF) through community events, outreach activities, volunteer opportunities, and other market-based programming.

The Community Engagement Coordinator is responsible for supporting a thoughtful and intentional process for tracking SMVF participation, engagement, and follow-up activities before, during, and after events throughout the market. Through close collaboration with internal teams and community partners, this role helps strengthen outreach efforts, improve visibility of available resources, and promote meaningful connections to supportive services and programs.

This position supports the management and utilization of technology platforms and data systems to document, track, and analyze SMVF engagement and participation trends. The Coordinator implements best practices and standardized procedures to maintain accurate and timely data entry, ensuring the integrity of participant records, communication efforts, demographic information, and follow-up activities related to care coordination and resource connection.

Using dashboards, tracking tools, meetings, and supervision, the Community Engagement Coordinator helps prioritize outreach efforts and supports successful engagement strategies that promote connection, wellbeing, and long-term support for SMVF within the market.

GENERAL RESPONSIBILITIES

· Lead community engagement efforts to achieve goals outlined in the annual Veterans Bridge Home plan and funding requirements by identifying and engaging Service Members, Veterans, and their Families (SMVF) through events, outreach activities, and community-based programming.

· Plan, coordinate, resource, attend, facilitate, and support community engagement events in collaboration with market teams and strategic partners to increase awareness of available resources and services, including mental health and suicide prevention resources, while promoting the health and wellbeing of SMVF.

· Engage SMVF at community events and through referrals received in Unite Us from NC Serves, providers, and SMVF self-referrals; conduct brief intakes (in person or by phone) including suicide screening to identify needs and determine appropriate support, referrals, and follow-up actions.

· Following intake and screening, refer SMVF to appropriate community resources, send resource information directly to clients, and consult or collaborate with Care Coordination/Navigation teammates for additional assessment, specialized screening, or connection to emergency/crisis resources when appropriate.

· Ensure eligible veterans are successfully connected to the U.S. Department of Veterans Affairs (VA) and Veterans Health Administration (VHA).

· Provide timely, person-centered follow-up through phone calls, emails, and personalized outreach to re-engage SMVF in community events, social enrichment opportunities, and partner programs.

· With foundational knowledge of health and human service domains such as employment, housing, financial assistance, and behavioral health, connect Service Members, Veterans, and their Families (SMVF) to holistic community resources through timely, accurate, and secure referrals to trusted providers using VBH technology platforms and established best practices.

· Track, manage, and analyze SMVF attendance, demographics, referrals, engagement activity, and follow-through outcomes to support effective outreach strategies and promote timely connection to services.

· Manage data and technology related to referrals, open cases, pending actions, and partner engagement while ensuring accurate documentation and adherence to standardized procedures and best practice guidelines.

· Maintain accurate and timely SMVF interactions and follow up activities within organizational technology platforms.

· Assist the Market President (MP) and market teams in developing and strengthening a culture of collective impact and community collaboration by leveraging existing partnerships and supporting community-based initiatives.

· Support the identification, development, and engagement of community providers by promoting military cultural competency, Mental Health First Aid, suicide prevention training opportunities, and tools that improve service delivery and outcomes for SMVF.

· Support the recruitment, training, engagement, and management of VBH Captains and volunteers to strengthen community outreach and Veteran engagement efforts across the VBH geographic footprint.

· Adhere to organizational procedures, training standards, best practices, and volunteer management expectations established by Veterans Bridge Home leadership.

· Conduct oneself in a manner consistent with the professional standards and values of Veterans Bridge Home while contributing positively to organizational culture and team collaboration.

GENERAL QUALIFICATIONS

To perform this job successfully, an individual must meet the following criteria. Reasonable accommodation may be provided to enable otherwise qualified individuals with disabilities to perform essential functions.

· 1–2 years of professional experience in community engagement, outreach, event planning, customer service, sales, case management, human services, or a related field; or an equivalent combination of work experience, training, military experience, and education. Bachelor’s Degree preferred (Social Sciences or related field preferred).

· Veteran or military spouse preferred or demonstrated understanding and application of military cultural competency.

· Demonstrated commitment to ethical practice, cross-cultural awareness, and alignment with the mission and values of Veterans Bridge Home.

· Excellent written and verbal communication skills, including the ability to effectively present, facilitate, and communicate in both in-person and virtual environments.

· Demonstrated initiative, accountability, strategic thinking, and attention to detail, with the ability to work independently, adapt to changing priorities, and manage responsibilities in a fast-paced environment.

· Ability to recognize challenges, identify solutions, and communicate effectively up, down, and across the organization to support the needs of Service Members, Veterans, and their Families (SMVF).

· Strong organizational and time management skills, with the ability to manage multiple priorities, coordinate events and outreach activities, and effectively utilize available resources.

· Excellent interpersonal and relationship management skills, with the ability to professionally engage and collaborate with community partners, volunteers, internal teams, and diverse stakeholder groups.

· Ability to work collaboratively across diverse health and human service settings and support a culture of collective impact and community partnership.

· Demonstrated ability to establish goals, measure engagement efforts, and articulate the value and impact of outreach activities and community relationships.

· Ability to work autonomously, navigate ambiguity, and exercise sound judgment in complex or evolving situations while maintaining professionalism and accountability.

· Proficiency with Microsoft Office Suite, Teams, SharePoint, and web-based applications, with the ability to learn and effectively utilize new technology platforms used by Veterans Bridge Home.

· Valid driver’s license, reliable transportation, and current automobile insurance required.
