Position Summary:
As a Qualified Mental Health Professional-Community Services (QMHP-CS), you will play a pivotal role in engaging and empowering children and their families by delivering weekly, tailored skills training and targeted case management services, offered both face-to-face and virtually. Working closely with our collaborative clinical team, you will help design personalized treatment plans that leverage client strengths to address specific behavioral challenges. In this role, you will act as a resourceful advocate, assisting families in navigating both routine and intensive case management activities while connecting them with essential community resources. By utilizing a strength-based approach and your QMHP expertise, you will implement a service model that fosters hope, builds confidence, and cultivates lasting, positive change in the lives of those we serve.What You'll Do:
Case Management:
- Deliver weekly skills training and targeted case management services tailored to each child and family's unique needsManage case assignments, develop service plans, track progress, and execute case closuresAssist families in navigating case management (routine/intensive) activities, connecting them with essential resources and support systemsTrack and record case information, complete forms, and produce reportsFacilitate care and treatment via scheduling, information sharing, and case coordinationAdvocate for clients with advice and information about treatment options and clinical status, evaluating medical necessity and appropriating resourcesWork closely with our clinical team to create personalized service/treatment plans that leverage strengths and address specific behavioral challenges
Policy and Compliance:
- Timely and accurate completion of case notes in full compliance with Medicaid regulations (48 hours)Maintain up to date knowledge of relevant regulations and ensure compliance with all applicable federal, state and local laws and regulationsAdhere to all organizational expectations, responsibilities, policies, and procedures as outlined for all staff members
Minimum Qualifications:
- Bachelor’s degree in Nursing, Special Education, Psychology or healthcare related disciplineCurrent HIPAA and QMHP certification, CAN 3-5/CAN 6-17/ANSA and others as requestedOne (1) year in a mental health clinical setting working with all age groupsProven proficiency in psychological aspects of patient care conducting comprehensive assessments, developing accurate diagnoses, and creating effective treatment plans. Familiarity with local community resources and social service systemsAdvanced skills in implementing a service model that fosters hope, builds confidence, and cultivates lasting positive change. Proficient organizational and critical thinking skills. Proficiency in clinical documentation within electronic health record (EHR) systems. A strong commitment to working with children with severe emotional disturbances and their families, leading with empathy and understandingExcellent interpersonal and communication skills (both verbal and written) to effectively communicate with patients, families, and multidisciplinary teams. Demonstrated ability to manage time effectively, prioritize tasks and maintain accurate records. Flexible availability to service clients both virtually and in-person, as needed. Competency in using computer systems and Google Workspace (formerly G Suite).
