Job Description

Responsibilities:

Under the supervision of the Supervisor of Independent Skills Training, provide a range of services and supervision to residents of West 108th Street to assist them to identify strategies to increase their self-management of personal and instrumental activities of daily living.

SERVICES TO RESIDENTS

• Train and coach residents as needed in activities of daily living and independent life skills, including personal care, room cleaning, laundry, budgeting and bill-paying, shopping, and socialization.

• Escort residents to medical, entitlement, and other appointments as directed.

• As needed, help residents with room-clean-outs and preparation for bed-bug treatments, including but not limited to packing up clothes and other items and doing laundry. While working on the bed bug prep, the Trainer will, in conjunction with other staff, help the resident understand and cope with this intrusive process.

• Shop for program supplies as directed.

• Run resident groups and activities as requested.

• Assist residents in utilizing in-house programs--activity groups, floor meetings, outings, socials, etc. This includes outreach to residents and assisting with the set-up and clean-up of an activity.

• Make visits to residents in hospitals, nursing homes or other facilities as directed.

• Work as a member of the team to establish and maintain high level of care and respect for the residents.

• Complete documentation of services provided as required.

• Work with other staff to insure good communication with tenants.

• Work cooperatively with other staff members.

• Share information about tenant progress, needs, and problems with staff as appropriate.

• Attend training sessions and conferences as required for enhancement of job skills.

• Implement emergency procedures as necessary.

• Other duties as assigned.

QUALIFICATIONS EXPECTED:

• HS Diploma or equivalent

• Experience in working with special needs population

• At least 21 years of age.

• Emotionally, mentally, and physically able to perform job responsibilities

• Able to speak, read, and write English (as required by DSS regulation 487.9.13)
