About This Position

Position Title: Pharmacist - Ambulatory (.6 FTE)

Description

Wage Range: $80.09 - $108.11 per hour

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.

Job Summary:

Responsible for the provision of pharmaceutical care to patients at EvergreenHealth ambulatory clinic locations. Prescribes and manages disease state specific pharmacotherapy and assumes medication management responsibilities at the request of, and in collaboration with, other EvergreenHealth providers.

Primary Duties:

  1. Collects and reviews patient history. Reviews the medical record using a problem-oriented framework in order to prioritize the medication related needs. Performs a comprehensive medication review. Solicits information from the patient, family, and caregivers to identify all current medications, including supplements and OTC products and to understand adherence patterns. Collects allergy information. Provides detail regarding any medication allergies. Collects and organizes other patient data.
  2. Assesses and evaluates the patient. Performs a limited physical exam. Assesses the appropriateness of current medications with regard to indication and therapeutic goals. Assesses the appropriateness of current medications with regard to effectiveness, safety, and cost. Assesses adherence patterns. Identifies any medication related issues which could lead to adverse outcomes if an intervention is not made.
  3. Develops and implements a plan of care in collaboration with other healthcare providers. Provides an individualized treatment plan to optimize medication therapy in collaboration with other providers. Identifies patient specific outcomes and goals. Determines the appropriate interval for follow-up. Reviews the medical record for updates on the patient's clinical status and progress. Identifies any new medication related problems or concerns. Conducts ongoing assessments with refinements to the plan of care with the goal of optimizing care and achievement of the treatment goals. Institutes measures essential for the life of the patient.
  4. Orders any necessary laboratory tests required to initiate or evaluate medication response and effectiveness, hepatic metabolism, renal clearance, or to assess for possible medication side effects.
  5. Orders diagnostic tests required to evaluate response to therapy or to contribute to risk assessment and treatment decisions as defined by the applicable CDTA or consultation request.
  6. Prescribes and monitors pharmacotherapy within the parameters of the applicable CDTA or consultation request. Integrates provided services within the patient's broader healthcare treatment plan. Documents all care provided and interventions prescribed or provided in the medical record and communicates essential information as necessary to other providers.
  7. Provides education to patients, families, and caregivers regarding the diagnosis, treatment course, non-pharmacologic interventions, and the safe and appropriate use of prescribed medications. Education strategies will be tailored to the patient's needs in order to facilitate understanding. Family and caregiver education will be utilized to improve understanding and adherence. Non-pharmacotherapy education will be provided to enhance effective treatment of the disease state. Examples of this education include smoking cessation counseling, and lifestyle modification strategies.
  8. Provides oversight of population level prescribing and pharmaceutical care trends for interventions required by quality measures, cost containment initiatives, or necessary for improved patient care as identified by pharmacy and clinic leadership.
  9. Performs other duties as assigned.

REQUIRED for the position

  • Washington state pharmacist license
  • Bachelor of Pharmacy or Doctor of Pharmacy degree
  • Current certification or active participation in the examination process leading to board certification. New hires are required to obtain at least one (1) board certification within 2.5 years of hire. Approved board certifications include:

o Certified Anticoagulation Care Provider (CACP)

o Certified Lipid Specialist (CLS)

o Certified Pharmacotherapy Specialist (BCPS)

o Certified Ambulatory Care Provider (BCACP)

o Certified Geriatric Pharmacist (CGP)

o Tobacco Treatment Specialist Certification (RMTTS-C)

  • Completion of the EvergreenHealth Credentialing process within 4 months of hire
  • Current American Heart Association Healthcare Provider BLS certification by date of hire
  • Satisfactory completion of applicable Collaborative Drug Therapy Agreement training

DESIRED for the position:

  • Preceptor license
  • Resident training or equivalent experience
