JOB TITLE: Clinical Pharmacist

POSITION SUMMARY: Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care across inpatient, swing bed, emergency department, and outpatient infusion services at Snoqualmie Valley Hospital. This role supports safe, effective, and evidence-based medication use through direct patient care, interdisciplinary collaboration, and operational pharmacy services.

ESSENTIAL FUNCTIONS:

• Provide patient-centered pharmaceutical care with a focus on safety, accuracy, and ethical practice
• Communicate clearly and respectfully with patients, families, providers, and care teams regarding medication therapy decisions
• Ensure compliance with all state, federal, and hospital regulations, policies, and procedures
• Promote a culture of safety by identifying, reporting, and addressing medication errors, near misses, and system risks
• Maintain accurate and timely documentation of clinical interventions, medication monitoring, and patient care activities

• Perform comprehensive medication therapy management, including selection, dosing, monitoring, and optimization of drug therapy
• Manage and monitor high-risk therapies (e.g., anticoagulation, vancomycin, TPN, infusion therapies) to ensure best outcomes
• Participate in multidisciplinary rounds and proactively contribute to clinical decision-making
• Lead and support quality improvement initiatives, medication use evaluations, and performance metrics
• Maintain clinical competency through ongoing education, literature review, and application of evidence-based practices

• Support development and expansion of new clinical services, including infusion therapies and future ambulatory and retail pharmacy services
• Contribute to implementation of new workflows, technologies, and care models to improve medication use and operational efficiency
• Adapt to evolving organizational needs and provide cross-coverage across inpatient, ED, infusion, and clinic settings
• Participate in building and refining clinical protocols, order sets, and pharmacy-driven initiatives
• Identify opportunities to enhance pharmacy services in a growing Critical Access Hospital environment

Deepen Connections: We build strong relationships with our patients, their families and partners in the communities we serve.

• Collaborate closely with providers, nurses, and interdisciplinary teams to deliver coordinated, high-quality care
• Provide education and counseling to patients and families to improve medication understanding and adherence
• Build strong working relationships across inpatient, outpatient, and community care settings
• Serve as a trusted clinical resource for medication-related questions and therapeutic guidance
• Support community engagement and represent the pharmacy department in outreach and partnership efforts

QUALIFICATIONS:

Required Education:

• Doctor of Pharmacy (PharmD) or equivalent

Required Licensure:

• Active Pharmacist license in the State of Washington

Required Experience:

• Demonstrated experience in the following areas: Anticoagulation therapy, bridge therapy, TPN, Vancomycin dosing/monitoring, drug information, and demonstrated teaching ability.

Preferred Experience:

• Prior inpatient hospital experience is preferred; however, candidates with strong clinical training and a desire to practice in a broad, team-based Critical Access Hospital environment are encouraged to apply.
• Demonstrated ability regarding bedside manner with patients and communication skills with patients, their families and health care professionals.
• Current in prescribing trends, new drugs and drug treatment developments in the anticoagulation clinic setting.
• Demonstrated skill in developing and maintaining positive interpersonal relationships with a diverse population.
• Participates in and supports professional organizations.
• Ability to communicate and work effectively with the physical and emotional developments of all age groups.

Benefit Eligibility Requirements - Employees regularly scheduled at least 20 hours per week.
