About Coastal Oaks Recovery
Located in the heart of the Lowcountry in South Carolina, Coastal Oaks Recovery is a 72-bed substance-use treatment center serving adults aged eighteen and older. Nestled on five serene acres near the coastal waters and wetlands of Port Royal, Coastal Oaks provides a tranquil environment for individuals suffering from addiction to stabilize and embark on their journey toward recovery and wellness.
At Coastal Oaks Recovery, we offer medically monitored detoxification under the direct care of physicians and nurses, supported by a multidisciplinary treatment team. In addition to withdrawal management, our residential treatment services provide a safe and structured environment for 30 to 45 days, ensuring that each patient builds a solid foundation of recovery skills prior to discharge.
Our evidence-based programming includes group therapy, individual counseling, family programming, and specialized care tailored to meet individual needs. With a holistic and experiential approach, we emphasize safety, collaboration, and a patient-first culture that nurtures recovery and wellness for both patients and staff.
Position Purpose
The Case Manager provides comprehensive case management services to patients during their treatment at Coastal Oaks Recovery. This role involves conducting evaluations, formulating individualized care plans, coordinating discharge planning, and collaborating with a multidisciplinary team to ensure seamless transitions and continuity of care. The Case Manager plays a critical role in advocating for patients and connecting them with community resources to support sustained recovery.
Essential Duties and Responsibilities
Patient Assessment and Planning
- Conduct thorough assessments of patients’ psychosocial needs, strengths, and barriers to recovery.
- Develop and implement individualized care plans that address treatment goals and discharge planning needs.
- Collaborate with patients, families, and the treatment team to create a comprehensive discharge plan, including referrals to community resources, housing, and employment support.
Collaboration and Advocacy
- Act as a liaison between patients, families, and community resources to ensure continuity of care.
- Educate patients and families about the recovery process, including their rights and responsibilities.
- Advocate for patients’ needs, including access to medical, legal, and financial resources.
- Work closely with the clinical team to provide holistic, patient-centered care.
Discharge Coordination and Follow-Up
- Coordinate discharge plans, ensuring all necessary resources and supports are in place for patients’ transitions.
- Arrange follow-up care and aftercare services, including outpatient treatment and support groups.
- Monitor and evaluate the effectiveness of discharge plans, making adjustments as necessary.
Documentation and Compliance
- Maintain accurate and timely documentation of all case management activities in accordance with Coastal Oaks Recovery policies and procedures.
- Ensure compliance with HIPAA and confidentiality regulations.
- Regularly update patient records to reflect progress and changes in treatment plans.
Skills and Abilities
- Strong communication, organizational, and interpersonal skills.
- Ability to work effectively in a multidisciplinary team environment.
- Knowledge of substance use disorders, co-occurring conditions, and evidence-based treatment approaches.
- Proficiency in electronic medical records (EMR) systems.
- Ability to manage challenging situations with empathy and sound judgment.
Education and Experience
- Required: Associate degree in Behavioral Science, Psychology, Sociology, or a related field.
- Preferred: Bachelor’s degree in a related field.
- Minimum of one year of experience in case management or counseling within a substance use treatment or mental health setting.
- Familiarity with community resources and discharge planning processes.
Licensure/Certification
- Current CPR and First Aid certification.
- HIV/AIDS training (required every two years).
- Case Management Certification is preferred.
- Additional certifications related to substance use treatment are a plus.
Physical and Work Environment Requirements
- Frequent standing, walking, bending, and sitting.
- Ability to lift up to 20 pounds occasionally.
- Work primarily indoors in a temperature-controlled environment.
- Ability to maintain professionalism and empathy in high-stress situations.
