Help residents stay housed, access resources, and build community.
BHNC is seeking a compassionate, organized Case Manager/Service Coordinator to support adults, older adults, people with disabilities, and families at Bernal Gateway Apartments. This role combines individualized support with resident engagement to strengthen housing stability, wellbeing, and belonging.
What You’ll Do
Conduct intakes and needs assessments, develop individual service plans, and follow through on resident goals and referrals.
Connect residents to benefits, food assistance, healthcare, transportation, legal resources, employment, and family services.
Help residents navigate lease requirements, recertifications, rent arrears, and resources to prevent eviction.
Coordinate with property management and community providers while protecting resident confidentiality.
Respond to crises within your training and role, connecting residents to appropriate support.
Organize resident meetings, workshops, community events, and leadership opportunities.
Conduct outreach and maintain accurate service records, attendance data, and outcome documentation.
What You Bring
Strong communication, organization, follow-up, and conflict resolution skills.
Experience with resident assessments, service planning, resource navigation, and confidential documentation.
Knowledge of San Francisco community resources and a commitment to culturally responsive services.
Preferred but not required; fluency in spoken and written Spanish and effective communication in English.
Preferred: Three or more years of case management or service connection experience; affordable housing or community engagement experience; a related college degree; and familiarity with Salesforce, GetCare, Asana, or similar systems.
Schedule and Benefits
Monday–Friday, generally 9 a.m.–5 p.m., with occasional evening or weekend activities and travel to other BHNC sites.
