DUTIES AND RESPONSIBILITIES:

The MH/DA Casemanager is responsible for the daily mental health needs of the Possibilities residents; proper preparation and maintenance of required documentation and records as well as close coordination with other services and agencies. Because of the nature of this Agency, it is essential that the MH/DA Casemanager believe in the philosophy of the Agency as well as be able to develop a rapport with Possibilities program participants.

CRITICAL ELEMENTS:

1) Assess Needs and Referrals: Assess individual mental health needs and make referrals to mental health services. Assess Drug and Alcohol Intervention needs and make appropriate referrals. Assess children’s needs and make appropriate referrals.

2) Provide Guidance: Provide supportive guidance and advocacy for the MH/DA needs of the program participants including crisis intervention.

3) Service Coordination: Attend CYOP meetings and court hearings as scheduled.

4) Casework Staff: Make recommendations to casework staff regarding client mental health needs.

5) Transport Residents: Transport residents to meet their needs in accomplishing program goals.

6) Workshops: Design and instruct or arrange mental health, drug and Alcohol related, and life skills workshops for program participants.

ADDITIONAL ELEMENTS:

1) Develop and assist in the implementation of individualized client-centered goal plans.

2) Monitor and evaluate assigned clients’ progress throughout their placement (goal plans, evaluations, apartment visits, etc.)

3) Provide client networking with community resources to assist clients toward stabilized living.

4) Assist clients, prior to discharge, with a discharge plan (housing search, landlord reference and community networking/referrals).

5) Take accepted clients through Possibilities Housing intake process.

6) Provide aftercare services as required or needed.

7) Assist with the planning and implementation of monthly recreational activities for children and adults.

8) Assist committee in processing potential families.

9) Maintain up-to-date client records.

10) Participate in staff meetings, house meetings, and appeal hearings as needed and assigned trainings.

11) Assist in the periodic review of Possibilities Housing’s programmatic format.

12) Assist in maintaining Possibilities Housing statistics.

13) Assist in the general upkeep of the Possibilities Housing facilities including monthly fire drills.

14) Attend 12 (twelve) hours of job related training each fiscal year.

15) Participate in Agency fund raising projects as required.

16) Follow and support Agency philosophy, policies and procedures.

17) Other duties as assigned by Program Coordinator.

Minimum Qualifications: Bachelor’s Degree in Social and two (2) years Human Service experience. Documented D&A Experience and training required. Pennsylvania State Police Background Check, ACT 33 Clearances. E.O.E.
