Summary of job:
As a Forensic Case Manager, you will accomplish clients care by assessing treatment and social service needs with particular focus on successful re-entry and follow-through with care in the community; developing, monitoring, and evaluating treatment plans and progress; and facilitating interdisciplinary approaches. This position provides case management to incarcerated individuals with co-occurring disorders transitioning to the community from Berks County Jail System. You will complete an assessment of the incarcerated individual’s strengths and needs and work with the individual to complete an individualized strength- based service plan to guide them in overcoming barriers to recovery and towards independence and stability. You must be able to work with an integrated team and collaborate with behavioral health and medical departments to meet the needs of the individual and identify appropriate resources in the community to facilitate the individual’s recovery.
You will…
- Assist individuals with transition from Berks County Jail System, inpatient facility, including rehab facility to coordinate continuance with outpatient services.
- Complete a comprehensive assessment of incarcerated clients’ skills, strengths, and needs to effectively determine clients’ needs
- Complete intakes on new clients when needed and initiate all pertinent paperwork, including a review of client’s rights and consent forms.
- 4Work cooperatively with clients and team to develop a unique service plan based on each client’s skills, strengths, and needs.
- Encourage, support, and assist clients in meeting support plan goals.
To qualify for the position, you will need…
- At least a High School Diploma or GED
- Two (2) years of experience in the social services field or related field required
- A valid PA Driver’s License and the ability to drive, travel, and provide community-oriented services.
- Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.
- CAAP, CAAC, CADC, or CAADC OR actively pursuing this certification during course of employment as evidenced by clinical supervision hours and completing required trainings.
Preferred candidates will …
- Bachelor’s degree in social services or related field
- Experience working with individuals with co-occurring disorders
- Be Bilingual: English/Spanish preferred.
- Knowledge of 12 Step Program and Community Resources
Physical Requirements:
- SEEING: Must be able to see well enough to read reports and use computer.
- HEARING: Must be able to hear well enough to communicate to clients, vendors, and coworkers.
- MOBILITY: Must have mobility to move about the jail, agency and the community.
- LIFTING/PULLING/PUSHING: Must be able to lift up to 5 pounds.
- OTHER: Must be able to write, type, and use technology.

A conviction is not a bar to employment; however, a review will be conducted to make a determination of employment should a conviction be present.
- Receipt of original documentation and verification of relevant degrees, diplomas, certificates, or licensure.
- Confirmation that you are not on the OIG Federal Healthcare Program Exclusion from Participation list. Should you appear on this list, Berks Counseling Center is prohibited from employing you.
- This position may require you to operate BCC’s company car or transport clients, so you must provide a 10-Year Motor Vehicle Record (MVR) and a valid Driver’s License
