Position Purpose:

The Manager Diabetes Program is a professional nurse or dietitian who oversees the Diabetes Program, provides consultation to patients and nursing personnel, and conducts and coordinates educational experiences. The responsibilities encompass assessment, planning, implementation, and evaluation of clinical skills development, in-services, and continuing education programs at the unit and/or department-level.

Key Job Activities:

• Responsible for day-to-day over site, supervision of program staff, daily operations and quality assurance for the Comprehensive Diabetes Health Education and Lifestyle Program.
• Directs the preparation and timely submission of all required reports and records to/for the Program, including those required for ADA certification and accreditation.
• Provides diabetes education to patients using materials and content supported by reputable clinical evidence.

• Education and Work Experience:

• Bachelor’s degree from an accredited school required, Master’s degree preferred.
• 3-5 years’ experience in diabetes education.
• Experience in the care of persons with chronic disease.

• Certifications/Licenses:

• Current valid license as a Registered Nurse or Registered Dietitian in the State of Illinois.

• Certified Diabetes Care and Education Specialist (CDCES).
