LPN Home Health in Hobart, Indiana at Powers Health

The Home Health Licensed Practical Nurse (LPN) evaluates patient/caregiver needs and provides skilled nursing care and education in collaboration with the Home Health RN and in accordance with a physician’s Plan of Care to meet the physical and psychosocial needs of the homecare patient. The LPN is responsible for the ongoing assessment, management and coordination of services provided to assigned homecare patients.

EDUCATION/EXPERIENCE:

• Graduate of an accredited school of nursing
• Current, valid professional IN state licensure
• Current, valid CPR certification
• One year experience as LPN in acute care setting
• Prior Home Health experience preferred
