Job Title: RN (SUD) Discharge Planner (Hagerstown)

Talantage, LLC is seeking an SUD Discharge Planner RN to join our team to complete discharge planning for patients referred to the multidisciplinary team based on the determination that community resources will be required at the time of release. This person will accomplish timely and appropriate discharge plans, act as a liaison between correctional healthcare teams and community agencies. This person will also collaborate and coordinate all parties regarding patient discharge planning. Day and evening shifts available.
Qualifications

• Active RN license to practice in Maryland
• Minimum of one year of experience as a Discharge Planner for a correctional or acute care facility
• Experience working in corrections preferred
• Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR)/ BLS
• Ability to obtain a security clearance, to include drug screen and criminal background check
