Job Title: RN Discharge Planner (Hagerstown)

Job Description:

Talantage is currently a Full-Time RN Discharge Planner to join our team. The RN Discharge Planner is responsible for coordinating patient discharge planning from a correctional setting. The Discharge Planner will work under the direction of the Director of Nursing in collaboration with a multidisciplinary team to develop and implement a safe and appropriate discharge plan to promote continuity of care after release.
Qualifications

• Current Registered Nurse in the state of Maryland or the ability to obtain license required
• Current BLS and CPR certifications required
• One-year experience as a Discharge Planner or Case Manager required
• Experience in corrections or an acute setting preferred
• Ability to obtain a security clearance to include drug screen and criminal background check
