Key Responsibilities

• Conduct initial nursing assessments and periodic reassessments for home care clients.
• Develop, review, and update individualized plans of care.
• Perform routine supervisory visits to monitor caregiver performance and ensure quality, compliant care.
• Assess clients' health status, identify changes in condition, and recommend appropriate interventions.
• Collaborate with physicians, caregivers, clients, families, and agency staff to coordinate care.
• Complete accurate, timely electronic documentation and required reports.
• Ensure compliance with Maryland home care regulations, agency policies, and professional nursing standards.

Qualifications

• Active Registered Nurse (RN) license in the State of Maryland.
• Minimum of one year of nursing experience; home health or home care experience preferred.
• Strong clinical assessment, critical thinking, and communication skills.
• Excellent organizational and documentation abilities.
• Reliable transportation and willingness to travel to client homes.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
