Job Summary
Facilitates cost-effective, quality patient transitions of care and outcomes by collaborating with staff and providing guidance, education, and support to assigned patients. Job Responsibilities
- Responsible for the coordination and facilitation of safe transitions of care.
- Assessment of complete chart and knowledge of patient's medical condition, financial status, emotional needs, and support systems.
- Collaborates with the Medical Director, Director, and Manager of Case Management on complex discharge transitions and delays of service.
- Interviews patients, assesses priorities and needs to develop a safe discharge plan.
- Works in collaboration with the social worker and Interdisciplinary team to ensure quality support of care and safety of discharge.
- Assists patients/families with referrals to community agencies and provides education and resource information.
- Provides timely and concise documentation in the medical record, the assessment, concerns, needs and discharge plan.
- Keeps plans up to date as patient progresses toward discharge.
- Provides education to physicians, staff, and patients/families.
- Supervises and/or trains other Case Managers, students or interns as needed.
- Associate's degree or higher or Diploma in Nursing required upon hire AND
- Bachelor's degree or higher in Nursing required within 5 years of employment in this position
- A minimum of 2 years' relevant experience required
- RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire
- Requires critical thinking skills and decisive judgment.
- Works under minimal supervision.
- Must be able to work in a stressful environment and take appropriate action.
