Admissions Coordinator
Classification: Full-Time, Non-Exempt
Reports to: Director of Marketing & Development
Position Summary:
The Admissions Coordinator facilitates communication between referral sources and the Admission Nurse(s). Based at the Henderson office, the Referral Coordinator coordinates referrals and admissions for both Palliative Care and all levels of Hospice care throughout the St. Anthony’s Hospice service area.
In addition to coordinating referrals, this role provides day to day operational direction to the Referral Nurses, ensuring smooth workflow, clear communication, and timely handling of all referrals. The Admissions Coordinator must have a solid understanding of the disease-specific criteria that makes a potential patient hospice appropriate, since they serve as the initial contact person for referral sources via phone, email, and fax. Strong organizational, leadership, and verbal communication skills are required in order to facilitate a smooth and efficient admission process whereby patient needs are fulfilled and agency goals are met.
Essential Job Functions:Serves as the primary point of contact for referrals and inquiries received by phone, email, and fax.
Provides day to day operational direction and support to Referral Nurses, including workflow coordination, prioritization of referrals, and problem resolution.
Documents all contacts and communications made during the referral and admission processes on the appropriate forms and communication tools at point of service.
Schedules and triages referral visits.
Gathers all needed medical records prior to referral visit being made and attaches to patient chart in EMR.
Collaborates with Leadership Team, Admission Nurse, and Referral Nurses.
Facilitates registration process and intake coordination.
Using a checklist, reviews admission charts for compliance (required signatures, all required forms present, supporting documentation present, admission criteria met, etc.).
Notifies SAH team of referrals received and scheduled and outcome of referral visit.
Tracks and follows up on all patients who are not admitted during initial Intake Visit.
Makes follow-up phone calls or sends secure messages to referral sources informing them of the outcome of the visit.
Responsible for working with Medical Directors and other physicians in the area to find attending physicians for new patients.
Assists with orienting and supporting new referral staff as needed.
Other duties as assigned.
Provides Assistance to: Director of Medical Services, Admission Nurse, Leadership Team, and Referral Nurses.
Professional Qualifications:High school diploma or GED required; post high school degree preferred.
If no healthcare experience, successful completion of course in Medical Terminology is required.
Strong interpersonal communication skills, verbal and written.
Self confident and at ease speaking to people from all sectors.
Highly organized with ability to multi-task, establish priorities, and pay attention to details.
Demonstrated understanding of disease-specific eligibility requirements and admission criteria for both general inpatient and home hospice care.
Demonstrated ability to guide, support, and direct others in daily operational work.
Must possess the personal qualities of responsibility, dependability, empathy, ability to positively influence others, and problem solve.
Strong familiarity with Internet navigation as well as demonstrated competency with Microsoft Word and Excel required.
Must be able to operate fax machine, copier, scanner, and other basic office equipment.
Must be able to constantly operate office equipment such as the copier, fax machine, calculator, computers, printers, and telephone system.
Must be able to be in a stationary position more than 50% of the time.
Must be able to move about the office to access files, office equipment, etc.
Must be able to communicate with employees, patients, families, providers, and vendors while coordinating the referral and admissions process, answering phones, sending emails, and participating in meetings.
Will frequently move files and objects up to 10 pounds.
