Position Title: Value-Based Primary Care Provider

We go beyond primary care with a full spectrum of services including cardiovascular health services, behavioral health, allergy testing and immunotherapy, in-house lab testing, imaging, chronic disease management, and other specialty health services. Job Responsibilities:
- Discuss and review patients' medical history, symptoms, allergies, and current medications
- Perform comprehensive physical examinations and clinical assessments
- Ask patients situation-specific questions to formulate accurate diagnoses
- Diagnose and treat acute and chronic medical conditions
- Develop and implement individualized treatment plans focused on improving patient outcomes
- Prescribe suitable medications and provide proper dosage and administration instructions
- Order, review, and interpret laboratory, imaging, and diagnostic test results
- Provide preventive care services, wellness visits, health screenings, and patient education
- Collaborate with care teams and specialists to coordinate comprehensive patient care
- Manage patient panels and support value-based care initiatives, quality measures, and population health goals
- Maintain accurate and timely documentation in eClinicalWorks (eCW)
Job Qualifications:
- Doctor of Medicine (MD), Nurse Practitioner (NP), or Physician Assistant (PA)
- Board-certified in Family Medicine, Internal Medicine, or a related field
- Active Arizona license or ability to obtain prior to start date
- Experience providing primary care services in an outpatient setting preferred
- Sound medical knowledge
- Experience utilizing electronic medical record systems, preferably eClinicalWorks (eCW)
- Knowledge of value-based care models, quality measures, and population health management preferred
- Excellent analytical and problem-solving skills
- Exceptional communication skills
- A patient and compassionate disposition
- Detail-oriented
