Social Services Coordinator in Sheboygan Falls, Wisconsin at Pine Haven Christian Home

Join the Pine Haven family! We are looking for a compassionate and experienced Social Services Coordinator to support the emotional and social well-being of our residents and their families. This position plays an important role in helping residents and ensuring they receive the support, resources, and quality care they deserve.

Essential duties and responsibilities

• Assist with psycho-social aspect of potential resident assessments
• Provide tours to potential residents and families
• Coordinate, and complete the admissions of new residents
• Responsible for providing residents and families with the social and emotional aspects of transitioning to our facility
• Meet with residents and families regarding emotional, social, or physical needs
• Assist staff with challenging resident or family situations
• Provide emotional support as resident needs arise
• Connect residents and families with community and government benefit programs and resources
• Coordinate, facilitate and participate in service plans, and complete follow up tasks from the care planning
• Investigate, document, and follow-up on resident rights concerns, including lost items
• Develop social services programs to provide higher level of care for residents, families, and staff
• Coordinate schedules for residents and outside providers who come on-site, such as podiatrist, audiologist, ophthalmologist/optometrist
• Work closely with support department staff and managers, including providing direction as needed to staff and sharing concerns with managers
• Serve on the Administrative On Call Rotation
• Provide Quality Assurance structure, review of programs, and implement QA initiatives as they are developed
• Follow infection control and universal precautions policies and procedures to ensure that a sanitary environment is always maintained
• Attend and participate in required training, conducted at time of hire and/or annually, that will include training designed to assure resident safety: i.e. fire safety, missing resident, evacuations, disaster response, etc.
• Report any observed changes in a resident’s needs and/or condition immediately to a license nurse or manager as appropriate

• Prior social services experience in a senior living setting required
• Ability to make independent decisions when circumstances warrant such action.
• Ability to deal professionally with personnel, residents, family members, visitors, government agencies/personnel and the general public.
• Patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever level they are currently functioning
• Ability to seek out new methods and principles and be willing to incorporate them into existing social services programs
• Maintain good attendance, including timely arrival
• Support positive working relationships with fellow team members
• Interreact, communicate, and collaborate effectively with others
• Attention to detail, time management, and organizational skills required
