About the role...
We’re also looking for someone who’s good at taking notes, able to prepare clinical documents, and assist with support and training for new case managers. Individuals with a background in addiction treatment or human services strongly preferred. Opportunities for growth/advancement widely available.
What you'll do...
- Ensure compliance with The Forge Recovery Center’s standards, policies, and code of ethics
- Uphold client rights and professional conduct
- Intervene in crisis situations as necessary
- Demonstrate proficiency in TAP21 Counselor Competencies
- Utilize agency assessment tools in KIPU for comprehensive client evaluations
- Identify and prioritize patient needs and issues
- Develop individualized care plans focusing on client-reported goals
- Conduct sessions using evidence-based practices
- Document client progress in SOAP notes
- Prepare effective discharge plans and transition notes
- Support and train Case Managers to help them improve their skills and knowledge
- Coordinate with health insurance for utilization reviews
- Participate in clinical treatment team meetings
- Provide accurate assessment and referral services
- Communicate effectively with probation officers, lawyers, and family members
- Adhere to ethical and legal standards
- Ensure accurate and timely clinical documentation
- Maintain awareness of cultural competency and client satisfaction
- Prioritize safety of staff and clients
Education, Experience, & Skills:
- Drug and Alcohol Counselor California Credential: CADC I, SUDCC II, CATC II or above
- 3-5+ years of Case Manager Experience
- Passion for client focused work and continuous improvement
- Ability to support and develop others
- CPR/First AID
- Valid Driver’s License
- Negative TB test results within past year
- Medical clearance to work within past year
Job Type: Full-time
Pay: $28.00- $32.00 per hour
