Compensation:
💰 $28 - $34 per hour – LPN's
💰 $42 - $47 per hour – RN's

Wellness Ranch, is a proud member of the Quadrant Health Group, is seeking compassionate and skilled Staff Nurses (LPN/RN) to join our team at our newest facility in Franklin, KY. What You'll Do:
The Nurse provides medical and nursing services to clients receiving treatment within the Wellness Ranch residential substance use disorder treatment program. The Nurse is responsible for monitoring client health status, administering medications, supporting detoxification services when applicable, and collaborating with the multidisciplinary treatment team to ensure safe and effective care.
The Nurse supports medical services for individuals receiving treatment at ASAM Levels 3.5 and 3.7/3.7-WM and assists in maintaining a safe, medically appropriate environment for clients.
Major Tasks, Duties and Responsibilities:
Nursing Care and Medical Monitoring
• Conduct nursing assessments upon client admission and throughout treatment as required.
• Monitor client health status and report significant changes to the medical provider and Director of Nursing.
• Observe clients for withdrawal symptoms and medical concerns when applicable.
• Provide basic nursing care and respond to medical concerns as they arise.
• Support the implementation of physician or medical provider orders.
Medication Administration
• Administer medications in accordance with physician orders and program policies.
• Maintain accurate medication administration records (MAR).
• Monitor clients for medication effectiveness and potential side effects.
• Ensure safe medication storage and handling in accordance with regulatory standards.
Client Health Education
• Provide basic health education to clients related to medication, withdrawal symptoms, and overall health.
• Encourage clients to participate in medical and wellness services during treatment.
• Support clients in understanding medical recommendations related to recovery.
Documentation
• Document all nursing services and observations in the electronic medical record (EMR) according to program policies and regulatory requirements.
• Maintain accurate records related to medication administration and medical monitoring.
• Complete incident reports and medical documentation as required.
Collaboration with Treatment Team
• Communicate relevant medical information to the clinical and behavioral health team.
• Participate in multidisciplinary treatment team meetings as appropriate.
• Coordinate care with medical providers and outside healthcare providers when necessary.
• Support implementation of individualized treatment plans.
Regulatory and Compliance Responsibilities
• Maintain client confidentiality in accordance with HIPAA and 42 CFR Part 2.
• Follow program policies and procedures related to medical care and medication administration.
• Support compliance with Kentucky behavioral health and substance use disorder treatment regulations.
• Maintain current licensure and participate in required training and continuing education.

• Knowledge of substance use disorders and withdrawal symptoms.
• Knowledge of medication administration and safe medication practices.
• Ability to recognize and respond to medical or psychiatric emergencies.
• Strong communication and documentation skills.
• Ability to work effectively within a multidisciplinary treatment team.
Minimum Qualifications
• Current Kentucky nursing license (RN or LPN) in good standing.
• Experience in behavioral health, detoxification services, substance use disorder treatment, or related healthcare setting preferred.
Required Clearances and Certifications
• Kentucky AOC background check
• OIG Exclusion List screening
• Drug screening as required by program policy
• TB screening as required by program policy
• CPR / First Aid certification (or BLS certification)
Work Environment
This position operates within a residential substance use disorder treatment facility and requires regular interaction with clients experiencing substance use and co-occurring mental health disorders.
