Job Description
We are seeking a Primary Care Physician to provide expert care management to patients in their homes, skilled nursing facilities, and group homes. This role is ideal for an advanced practice provider passionate about improving patient outcomes and quality of life through primary care and specialized care treatments.
As a Primary Care Physician, you will conduct physical examinations, interpret lab tests, oversee patients' health, diagnose and and collaborate with interdisciplinary healthcare teams. This role requires a strong understanding of disease prevention practices, helping people lead healthy lives, wound healing principles, infection control, and chronic disease management.
This position offers autonomy, flexibility, and the opportunity to work in a mobile healthcare environment, providing care to patients in Memphis, TN, and surrounding counties.
Responsibilities:
- Advanced professional knowledge
- Promote good health practices
- Oversee patients' overall health
- Educate patients and their family members on diseases, epidemics, and prevention
- Inform patients about their health conditions
- Propose treatments for chronic and infectious diseases
- Diagnose health conditions and illnesses
- Order and interpret diagnostic tests (blood tests, X-rays, etc.)
- Examine patients and their medical records
- Develop individualized treatment plans.
- Order and interpret diagnostic tests, lab work, and imaging studies.
- Educate patients and caregivers on disease prevention strategies.
- Prescribe appropriate medications for overall health, wound healing, and infection control.
- Collaborate with primary care providers, specialists, and home health agencies to ensure coordinated patient care.
- Maintain accurate medical records and document patient progress in compliance with healthcare regulations.
Skills & Qualifications:
- Proven experience as a Primary Care Physician in primary care, acute care, chronic care practices, home health, and geriatrics.
- Ability to work autonomously while collaborating with healthcare teams.
- Excellent communication and patient education skills.
- Strong problem-solving abilities and clinical decision-making skills.
- Valid Nurse Practitioner / Physician Assistant license.

Requirements:Proven experience as a Nurse Practitioner or Physician Assistant in primary care, home health, or geriatrics.
